Board of Nursing Disciplinary Actions Taken in August 2009
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 34 nurses or nursing assistants during its August 20, 2009 Board Meeting.
 
Baker County
  • The Board denied the CNA renewal application of Diana R. Lowrey (000042303CNA), of Baker City, Ore., due to a mental condition that prevents her from safely performing CNA duties.
Benton County
  • The Board revoked the Registered Nurse license of Helen C. Kellie China (200140640RN), of Corvallis, Ore., for violating the terms and conditions of a Board Order.
Clackamas County
  • The Board revoked the Registered Nurse license of Lynette E. Rodd (089007266RN), of Welches, Ore., for violating the terms and conditions of a Board Order and failing to cooperate with the Board during an investigation.
  • The Board accepted the voluntary surrender of Manuella J. Dwyer’s Practical Nurse license (200730271LPN) for client abuse, unauthorized removal of drugs from the workplace and aiding an individual to violate the law.  She resides in West Linn, Ore.
Clatsop County
  • The Board revoked the nursing assistant certificate of Lori A. Paich (200712477CNA), of Astoria, Ore., for violating the terms and conditions of a Board Order.
Crook County
  • The Board denied the nursing assistant application of Tiffanie J. Casteel, of Prineville, Ore., for Criminal Mischief and hit-and-run convictions, and misrepresentation during the application process.
Douglas County
  • The Board revoked the nursing assistant certificate of Wayne H. Jones (200510788CNA), of Roseburg, Ore., for violating the terms and conditions of a Board Order.
  • The Board revoked the nursing assistant certificate of Rachel L. Everhart (200112108CNA), of Roseburg, Ore., for an Official Misconduct conviction, and attempting to obtain controlled drugs without a prescription.
  • The Board reprimanded the nursing assistant certificate of Laura J. Geisel (200811980CNA), of Roseburg, Ore., with conditions, for failing to respect client rights and engaging in other unacceptable behavior in the presence of clients.
  • The Board accepted the voluntary surrender of Lisa C. Shaffer’s Registered Nurse license (200242682RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Gardiner, Ore.
Jackson County
  • The Board placed the nursing assistant certificate of Wendy M. Davis (200310797CNA) of Ashland, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Lane County
  • The Board denied the nursing assistant application of Christopher E. Bates, of Eugene, Ore., for Possession convictions, and misrepresentation during the application process.
  • The Board revoked the nursing assistant certificate of Kara L. Bosley (200610717CNA), of Salem, Ore., for using intoxicants to the extent she was unable to perform CNA duties safely and failing to cooperate with the Board during an investigation.
  • The Board revoked the Registered Nurse license of Deanna S. Fleming (093003098RN), of McKenzie Bridge, Ore., for failing to comply with the terms and conditions of the Nurse Monitoring Program.
Lincoln County
  • The Board accepted the voluntary surrender of Misty J. Smith’s nursing assistant certificate (200410237CNA) for falsifying a client record and aiding an individual to violate the law.  She resides in Otis, Ore.
Marion County
  • The Board suspended the nursing assistant certificate of Elvia M. Cervantes (200512548CNA), of Salem, Ore., indefinitely for failing to cooperate with the Board during an investigation.
  • The Board revoked the nursing assistant certificate of Liza A. Zolnikov (000035108CNA), of Hubbard, Ore., for violating the terms and conditions of a Board Order.
  • The Board reprimanded the Registered Nurse license of Pipre E. Lawler (089003129RN), of Keizer, Ore., with conditions, for incomplete recordkeeping, leaving an assignment without notifying appropriate personnel, and failing to conform to the essential standards of acceptable nursing practice.
Multnomah County
  • The Board imposed a $2,500 civil penalty against Maria Posteuca, of Gresham, Ore., of DBA Cornerstone Adult Foster Home, for employing a nurse without a procedure in place for checking the current status of that nurse’s license to ensure that only those nurses with a current, valid Oregon license be allowed to practice nursing.
Polk County
  • The Board reprimanded the Psychiatric Mental Health Nurse Practitioner certificate of Della Fogerson (200750110NP), of Salem, Ore., with conditions, for failing to maintain client records in a timely manner.
  • The Board revoked the Registered Nurse license of Jay F. McKay (096006036RN), of Independence, Ore., and imposed a $400 civil penalty, for failing to supervise persons to whom nursing tasks had been assigned, practicing nursing without a current license, and failure to conform to the essential standards of acceptable and prevailing nursing practice.
Wallowa County
  • The Board reprimanded the Registered Nurse license of Tammy L. Knorr (200643002RN), of Joseph, Ore., with conditions, for incomplete and inaccurate recordkeeping, performing acts beyond authorized scope and failing to conform to the essential standards of acceptable nursing practice.
Washington County
  • The Board denied the Practical Nurse endorsement application of Meredith M. Hartley, of Portland, Ore., due to unresolved discipline in another state and use of a controlled substance in a dangerous manner.
  • The Board accepted the voluntary surrender of Jeanine M. Harris’ Registered Nurse license (093000357RN) for unauthorized removal of drugs from the workplace and practicing nursing while impaired.  She resides in Hillsboro, Ore.
  • The Board accepted the voluntary surrender of Mary H. Watts’ Registered Nurse license (096007025RN) due to a physical condition that renders her unable to practice nursing safely.  She resides in Beaverton, Ore.
  • The Board accepted the voluntary surrender of Nicole A. Busse’s nursing assistant certificate (200310764CNA) for violating the terms and conditions of a Board Order.  She resides in Tigard, Ore.
Yamhill County
  • The Board denied the CNA reactivation application of Joanna Frechette (200211399CNA), of Newberg, Ore., due to a mental condition that prevents her from performing CNA duties safely.
  • The Board denied the LPN reactivation application of Cindy Criteser (093005171LPN), of Dundee, Ore., for misrepresentation during the licensure process, not meeting the practice hour requirement for licensure, and failing to answer questions truthfully.
  • The Board accepted the voluntary surrender of Elijah B. Keithley’s nursing assistant certificate (200510487CNA) for Criminal Mistreatment and Burglary convictions.  He resides in Yamhill, Ore.
Out of State
  • Nevada—The Board denied the RN endorsement application of Edward J. Herk, of  Dayton, Nev., due to previous discipline in Arizona, failure to answer questions truthfully and misrepresentation during the application process.
  • Washington—The Board reprimanded the Registered Nurse license of Lois M. Tenita (200441372RN), of Vancouver, Wash., with conditions, and imposed a civil penalty of $600, for improperly delegating tasks of nursing care, inaccurate and incomplete recordkeeping, and practicing nursing without a license.
  • Washington—The Board placed the nursing assistant certificate of Darron W. Marzolf (200912289CNA), of Ellensburg, Wash., on probation for two years, for using intoxicants to the extent that he was unable to perform CNA duties safely.
  • New Mexico—The Board accepted the voluntary surrender of Aimee M. Kouba’s Registered Nurse license (098000257RN) for unauthorized removal of drugs from the workplace, a mental condition that prevents her from practicing safely and practicing when impaired.  She resides in Albuquerque, N.M.
  • Kentucky—The Board accepted the voluntary surrender of Laura A. Petering’s Registered Nurse license and Adult Nurse Practitioner certificate (200641301RN/200650063NP) for using intoxicants to the extent she was unable to practice safely, and practicing while impaired.  She resides in Louisville, Ky.
