Board of Nursing Disciplinary Actions Taken in February 2009
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 47 nurses or nursing assistants during its February 11, 2009 Board Meeting.
 
Clackamas County
  • The Board denied the Registered Nurse application of Laura M. Smith of Lake Oswego, Ore., for using a controlled substance to the extent she is unable to practice nursing safely.
  • The Board revoked the nursing assistant certificate of Tamie M. Hill (200410388CNA) of Wilsonville, Ore., for failing to respect client rights and exploiting the client for personal gain.
  • The Board suspended the Registered Nurse license of Christine A. Love (000032742RN), of Damascus, Ore., for 30 days for client neglect and failing to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the Voluntary Surrender of Aaron S. Groom’s Registered Nurse license (200740142RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program.  He resides in Clackamas, Ore.
Crook County
  • The Board denied the nursing assistant application of Tammy L. Desmond of Prineville, Ore., for fraud during the licensure process.
Deschutes County
  • The Board reprimanded the Family Nurse Practitioner certificate of Joannie J. Miller (200250124NP) of La Pine, Ore., for incomplete recordkeeping.
  • The Board revoked the Registered Nurse license of Sharon G. Souders (200742756RN) of Redmond, Ore., for failure to respect client dignity, client neglect, and failure to conform to the essential standards of acceptable nursing practice.
Douglas County
  • The Board denied the nursing assistant application of Ericka S. Lent of Roseburg, Ore., for using a controlled substance to the extent she is unable to perform CNA duties safely.
Jackson County
  • The Board accepted the Voluntary Surrender of Kathy A. Praegitzer’s Registered Nurse license (200141449RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Medford, Ore.
Jefferson County
  • The Board accepted the Voluntary Surrender of Douglas H. Hansen’s Registered Nurse license (080046370RN) for using a controlled substance to the extent he was unable to practice nursing safely.  He resides in Madras, Ore.
Klamath County
  • The Board denied the Registered Nurse reactivation application of Katherine V. Hendricks (095000467RN) of Klamath Falls, Ore., and imposed a $5,000 Civil Penalty for misrepresentation during the licensure process, failure to answer questions truthfully, and practicing nursing without a current license.
Lane County
  • The Board denied the CNA renewal application of Jennifer L. Caso of Cottage Grove, Ore., for fraud during the licensure process.
  • The Board revoked the nursing assistant certificate of Atasha S. Nelson (000038476CNA) of Eugene, Ore., for misrepresentation during the licensure process and using intoxicants to the extent she cannot perform CNA duties safely.
  • The Board reprimanded the Family Nurse Practitioner certificate of Eunice Castillo-Ross (088006694N1), of Eugene, Ore., for failure to maintain client records, failure to implement the plan of care, and failure to communicate information regarding client status to members of the health care team.
Lincoln County
  • The Board accepted the Voluntary Surrender of Delphine Talnagi’s nursing assistant certificate (200112665CNA) for using intoxicants to an extent she is unable to perform CNA duties safely.  She resides in Newport, Ore.
Linn County
  • The Board suspended the Registered Nurse license of Christina L. Ford (200441271RN)), of Harrisburg, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board reprimanded the nursing assistant certificate of Rodney E. Casper (200211097CNA) of Albany, Ore., for failure to answer questions truthfully.
Malheur County
  • The Board accepted the Voluntary Surrender of Donna C. Linze’s nursing assistant certificate (200512311CNA) for violating the terms and conditions of a Board Order.  She resides in Ontario, Ore.
Marion County
  • The Board suspended the nursing assistant certificate of Shalla R. Scruggs (200313411CNA), of Salem, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board accepted the Voluntary Surrender of Deborah A. Abell’s Registered Nurse license (091006592RN) for inaccurate recordkeeping and failure to conform to the essential standards of acceptable nursing practice.  She resides in Salem, Ore.
Multnomah County
  • The Board denied the CMA renewal application of Darrel J. Pavlish (200320192CMA) of Portland, Ore., for fraud during the licensure process and failure to respect patient rights.
  • The Board suspended the nursing assistant certificate of Morgan A. Shaw (200612501CNA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board suspended the nursing assistant certificate of Martha M. Brownlee (200612720CNA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board suspended the nursing assistant and medication aide certificates of Margaret D. McLaren Gerhold (200310551CNA/200420014CMA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board suspended the nursing assistant certificate of Amanda Rice (200411088CNA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board reprimanded the Registered Nurse license of A. Therese Arkus (076036715RN), of Portland, Ore., for performing acts beyond her authorized scope, and failing to communicate information regarding the client’s status to members of the healthcare team.
  • The Board reprimanded the Practical Nurse license of Joseph Van Horn (085074398LPN), of Portland, Ore., for failure to respect client dignity and failure to take action to preserve client safety.
  • The Board placed the nursing assistant certificate of Kami Salisbury (000014336CNA) of Portland, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board accepted the Voluntary Surrender of Deena J. Hahn’s Registered Nurse license (079011438RN) for a mental condition that prohibits her from providing safe nursing practice.  She resides in Portland, Ore.
  • The Board accepted the Voluntary Surrender of Michelle A. Ayres’s medication aide certificate (200120055CMA) for failing to document medications and performing acts beyond her authorized duties.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Nicole S. Eubanks (200311786CNA) of Fairview, Ore., for a conviction for Attempted Assault in the First Degree.
Polk County
  • The Board suspended the nursing assistant certificate of Mark J. Broadus (200011167CNA) for 60 days, to be followed by probation for at least two years, for failure to respect client rights and dignity.  He resides in Independence, Ore.
  • The Board accepted the Voluntary Surrender of Jordan L. Wigington’s nursing assistant certificate (200712575CNA) for client abuse, violating clients’ privacy rights and failure to respect client dignity.  He resides in Dallas, Ore.
  • The Board accepted the Voluntary Surrender of Kathleen R. Hawley’s Registered Nurse license (086003147RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Independence, Ore.
Umatilla County
  • The Board suspended the Registered Nurse license of Jennifer L. McLean (200341840RN), of Pendleton, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Wasco County
  • The Board suspended the nursing assistant and medication aide certificates of Nancie M. Rivera (000006044CNA/000006044CMA), of The Dalles, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board accepted the Voluntary Surrender of Dawna M. Rose’s nursing assistant certificate (200810367CNA) for violating the terms and conditions of a Board Order.  She resides in The Dalles, Ore.
  • The Board reprimanded the nursing assistant certificate of Mary M. Hoffman (000028941CNA), of The Dalles, Ore., for client neglect and failure to perform CNA duties competently.
Washington County
Yamhill County
  • The Board suspended the nursing assistant certificate of Lisa L. Ray (000035102CNA), of Lafayette, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Out of State
  • Arkansas—The Board denied the Registered Nurse application of Sean C. Savage of Melbourne, Ark., for failing to answer questions truthfully and a mental condition that prevents him from practicing nursing safely.
  • Florida—The Board denied the Practical Nurse application of Marlene M. St. Val of Boca Raton, Fla., for failure to meet education requirements.
  • Washington—The Board placed the Registered Nurse license of Marcy C. Harris (200440336RN) of Vancouver, Wash., on probation for two years for using intoxicants to the extent she was unable to practice safely.
  • Texas—The Board suspended the Registered Nurse license of Barbara E. Carroll (200040110RN), of New Braunfels, Texas, indefinitely for practicing nursing while impaired.
  • Texas—The Board accepted the Voluntary Surrender of Jessica R. Weiner’s Practical Nurse license (200530194LPN) for client neglect and failure to conform to the essential standards of acceptable nursing practice.  She resides in San Antonio, Texas.
