The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 33 nurses or nursing assistants during its July 16, 2009 Board Meeting.
Clackamas County
- The Board accepted the voluntary withdrawal of Ryan T. Pagano’s (000032471CNA) CNA reactivation application for using intoxicants to the extent he was unable to perform CNA duties safely. He resides in Portland, Ore.
Columbia County
- The Board denied the nursing assistant application of Jennifer A. St. John, of St. Helens, Ore., for Assault and Criminal Mistreatment convictions.
Deschutes County
- The Board revoked the Registered Nurse license of Deena M. Moore (099000702RN) of Bend, Ore., for violating the terms of a Board Order.
- The Board suspended the Registered Nurse license of Laurie S. Waltosz (081046938RN), of Redmond, Ore., for 90-days, with conditions, for inaccurate recordkeeping, assuming duties when competency has not been established, and failing to conform to the essential standards of acceptable nursing practice.
- The Board accepted the voluntary surrender of Terry R. McBride’s Practical Nurse license (000006268LPN) due to a physical condition that renders him unable to practice nursing safely. He resides in Bend, Ore.
- The Board suspended the Practical Nurse license of Monika R. Stanko (200630307LPN), of Bend, Ore., for 30-days, with conditions, for inaccurate and incomplete recordkeeping, falsifying records and failure to conform to the essential standards of acceptable nursing practice.
Douglas County
- The Board revoked the nursing assistant certificate of Camille L K. Thompson (200612019CNA) of Roseburg, Ore., for violating the terms of a Board Order.
- The Board placed the nursing assistant certificate of Katharine M. Sperlich (000036142CNA) of Sutherlin, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Jackson County
- The Board imposed a $5,000 civil penalty against Alonzo Mattison, of Medford, Ore., a nurse imposter.
- The Board revoked the Practical Nurse license of Linda Schwanke (200130431LPN) of Ashland, Ore., for violating the terms of a Board Order.
Klamath County
- The Board revoked the nursing assistant certificate of Heather K. Owen (200610956CNA) of Klamath Falls, Ore., for violating the terms of a Board Order.
Lane County
- The Board denied the CNA renewal application of Jimmy R. Solano (200412396CNA), of Eugene, Ore., for using intoxicants to the extent he is unable to perform CNA duties safely, and failure to cooperate during an investigation.
- The Board denied the CNA renewal application of Chelsie A. Allen (200810883CNA), of Veneta, Ore., for convictions for Attempting to Elude a Police Officer and Criminal Driving While Suspended.
- The Board revoked the nursing assistant certificate of George R. Fleming (200712620CNA) of Springfield, Ore., for client neglect, engaging in sexual misconduct, failure to respect client rights and engaging in other unacceptable behavior.
- The Board accepted the voluntary surrender of Jean A. Connaughty’s nursing assistant certificate (000036224CNA) due to an Unlawful Manufacture of Marijuana conviction. She resides in Eugene, Ore.
- The Board accepted the voluntary surrender of Jacqueline A. Mahan’s Registered Nurse license (085075806RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Eugene, Ore.
Linn County
- The Board revoked the nursing assistant certificate of Sherie N. Clifton (200710460CNA) of Albany, Ore., due to an Endangering the Welfare of a Minor conviction, and failure to answer questions truthfully.
Multnomah County
- The Board denied the nursing assistant application of Antonio H. Bermudez, of Portland, Ore., for an Assault conviction and failure to cooperate during an investigation.
- The Board denied the Practical Nurse application of Robert L. Janes, of Portland, Ore., due to previous discipline in New York.
- The Board denied the Registered Nurse application of Christine A. Shelamer, of Portland, Ore., due to a physical condition that makes the licensee unable to practice nursing safely.
- The Board suspended the nursing assistant certificate of Sara E. Greenfield (000010942CNA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board placed the nursing assistant certificate of CNA applicant Frances J. Alvarez of Portland, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board placed the nursing assistant certificate of Mitzi J. Resue (200911900CNA) of Portland, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Washington County
- The Board suspended the nursing assistant certificate of Kimberly A. Schweikert (200110686CNA), of Hillsboro, Ore., for 90-days for misrepresentation during the licensure process and failure to answer questions truthfully.
- The Board revoked the Registered Nurse license of D’Nise R. Keller (000043446RN) of Sherwood, Ore., for violating the terms of a Board Order.
- The Board accepted the voluntary withdrawal of Katherine A. Coleman’s (093000544RN) RN reactivation application for misrepresentation during the licensure process, using intoxicants to the extent she was unable to practice nursing safely. She resides in Beaverton, Ore.
Yamhill County
- The Board revoked the nursing assistant certificate of Toni C. Ray (000038257CNA) of Willamina, Ore., for an Assault conviction, fraud during the licensure process and failure to answer questions truthfully.
- The Board accepted the voluntary surrender of Rose A. Anderson’s nursing assistant certificate (000027518CNA) for client abuse. She resides in Newberg, Ore.
Out of State
- Indiana—The Board denied the Registered Nurse application of Kurt W. Peterson, of Lawrenceburg, Ind., for misrepresentation during the licensure process and failure to answer questions truthfully.
- Utah—The Board denied the CNA/CMA renewal application of Stephanie S. McCumber (200512129CNA/200720031CMA), of Ogden, Utah, for use of a controlled substance to the extent she is unable to perform CNA or CMA duties safely.
- Washington—The Board reprimanded the Practical Nurse license of Morgen B. Crumpacker (200730056LPN) of Walla Walla, Wash., for falsifying an agency record, assuming duties without documented preparation for the duties and when competency has not been established, and failure to conform to the essential standards of acceptable nursing practice.
- California—The Board accepted the voluntary surrender of Paul N. Gioia’s Practical Nurse license (200530339LPN) for using intoxicants to the extent he was unable to practice nursing safely. He resides in Cathedral City, Calif.
- Washington—The Board suspended the Registered Nurse license of Katherine C. Smith (200341328RN), of Kelso, Wash., for 60-days for leaving a nursing assignment without notifying appropriate personnel and failure to conform to the essential standards of acceptable nursing practice.
