The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 23 nurses or nursing assistants during its March 12, 2009 Board Meeting.
Clackamas County
- The Board suspended the Practical Nurse license of Brandy L. Clem (200730419LPN), of Oregon City, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board accepted the voluntary surrender of Dana S. Hohman’s Registered Nurse license (098003194RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Estacada, Ore.
- The Board denied the RN renewal application of Tami L. Monroe-Gonroff (090000613RN) of Oregon City, Ore., for misrepresentation during the licensure process, using intoxicating substances to the extent she is unable to practice safely, failure to provide documents to the Board, and failure to cooperate with the Board during an investigation.
Columbia County
- The Board accepted the voluntary surrender of Susan E. Elbert’s Registered Nurse license (200441857RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Rainier, Ore.
Hood River County
- The Board suspended indefinitely the nursing assistant and medication aide certificates of Kathryn M. Sottile (200611683CNA/200720116CMA) of Hood River, Ore., for failure to cooperate with the Board during an investigation.
Josephine County
- The Board revoked the nursing assistant and medication aide certificates of Traci A. Powell (000027991CNA/ 200220144CMA) of Grants Pass, Ore., for misrepresentation during the licensure process, and violation of a Board Order.
- The Board revoked the nursing assistant certificate of Troy L. Winger (200710068CNA) of Grants Pass, Ore., for a 3rd degree Rape conviction, fraud during the licensure process, and failure to answer questions truthfully.
Lane County
- The Board accepted the voluntary surrender of Michael P. Ritchey’s Registered Nurse license (097003197RN) for using drugs and alcohol to the extent he is unable to practice nursing safely. He resides in Eugene, Ore.
- The Board revoked the nursing assistant certificate of Jonathan G. Sanchez (200710164CNA) of Junction City, Ore., for using alcohol to the extent he is unable to perform CNA duties safely.
Linn County
- The Board accepted the voluntary surrender of Cassandra J. Hunsaker’s Registered Nurse license (200541191RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Lebanon, Ore.
Malheur County
- The Board denied the nursing assistant application of Gale G. Hayes of Ontario, Ore., for client abuse, sexual misconduct, failure to answer questions truthfully and failure to cooperate with the Board during an investigation.
Marion County
- The Board revoked the nursing assistant certificate of Estar E. Cummings (000015103CNA) of Keizer, Ore., for client neglect, failure to respect the rights of the client’s family, engaged in unacceptable behavior toward a client.
- The Board revoked the nursing assistant certificate of Shalla R. Scruggs (200313411CNA) of Salem, Ore., for a conviction for Criminal Mistreatment, failure to answer questions truthfully and failure to cooperate with the Board during an investigation.
Multnomah County
- The Board suspended the Practical Nurse license of Demetria Y. Adams (200130244LPN), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board revoked the Practical Nurse license of John H. Reeder (094005140LPN), of Portland, Ore., for client neglect, unauthorized removal of narcotics from the workplace, inaccurate recordkeeping, practicing while impaired, and failure to cooperate with the Board during an investigation.
- The Board accepted the voluntary surrender of Kenneth N. Verdier’s Practical Nurse license (097005159LPN) for a physical condition that prevents him from practicing nursing safely. He resides in Portland, Ore.
Polk County
- The Board suspended the nursing assistant certificate of Carlton D. Fuller (200710104CNA) of Monmouth, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Umatilla County
- The Board suspended the nursing assistant certificate of Jennifer L. Perkins (000029447CNA), of Milton Freewater, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Washington County
- The Board suspended the nursing assistant certificate of Bernie T. Bernard (200310143CNA), of Tualatin, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board accepted the voluntary surrender of Angela D. Dunkeson’s Registered Nurse license (200242928RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Forest Grove, Ore.
Yamhill County
- The Board accepted the voluntary surrender of Sheldon L. Hinshaw’s nursing assistant certificate (000007465CNA) for failure to respect client rights and dignity. He resides in McMinnville, Ore.
Out of State
- Colorado—The Board suspended the Registered Nurse license of Barbara A. Gentner (096006895RN), of Loveland, Colo., for a minimum of 60-days for jeopardizing patient safety, incomplete recordkeeping, performing acts beyond authorized scope, and failure to conform to the essential standards of nursing practice.
- Washington—The Board imposed a civil penalty on Practical Nurse applicant William Winkler, of Vancouver, Wash., for practicing nursing without a current license.
