The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 22 nurses or nursing assistants during its May 15, 2008, Board Meeting.
Benton County
- The Board suspended the Registered Nurse license of Barbara E. Carroll (200040110RN) of Corvallis, Ore., for use of a controlled substance to the extent she is unable to practice safely and for practicing while impaired.
Clackamas County
- The Board reprimanded the Registered Nurse license of Jana Clancy (000030341RN) of Wilsonville, Ore., inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of nursing practice.
Coos County
- The Board denied the nursing assistant application of Shannon D. Nordholm for failing to answer questions truthfully and misrepresentation during the certification process. She resides in North Bend, Ore.
Deschutes County
- The Board accepted the Voluntary Surrender of Rachel M. Carter’s Registered Nurse license (200541849RN) for incomplete and inaccurate recordkeeping and failing to conform to the essential standards of acceptable nursing practice. She resides in Bend, Ore.
Douglas County
- The Board reprimanded the Registered Nurse license of Angela E. Yow (200542074RN) of Myrtle Creek, Ore., for failing to answer questions truthfully and engaging in unacceptable behavior towards a client.
- The Board reprimanded the Registered Nurse license of Lori A. Hill (087003272LPN) of Roseburg, Ore., for performing acts beyond her authorized scope and failing to conform to the essential standards of acceptable nursing practice.
Hood River County
- The Board accepted the Voluntary Surrender of Richard E. Dant’s Registered Nurse license (200140246RN) for a mental condition that prevents him from practicing safely. He resides in Hood River, Ore.
Jackson County
- The Board accepted the Voluntary Surrender of David M. Coryell’s Registered Nurse license (098000656RN) for use of a controlled substance to the extent he is unable to practice safely. He resides in Central Point, Ore.
Lake County
- The Board revoked the Practical Nurse license of Lana R. Trawallay (200630384LPN) of Lakeview, Ore., for client abuse, failing to respect client dignity and failing to conform to the essential standards of acceptable nursing practice.
Lane County
- The Board revoked the nursing assistant certificate of Trista M. Thill (200110990CNA) of Eugene, Ore., for using the client relationship for personal gain.
Marion County
- The Board suspended the nursing assistant certificate of Clifford J. Hollingsworth (200711099CNA) of Salem, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Christian N. Florin (200411634CNA) of Salem, Ore., for failing to cooperate with the Board during an investigation.
- The Board revoked the nursing assistant certificate of Rebecka J. Kaylor (200010962CNA) of Silverton, Ore., for client abuse, threatening a coworker, and failing to respect client dignity and rights.
- The Board revoked the nursing assistant certificate of Deana M. Mabbitt (200710764CNA) of Salem, Ore., for a 3rd Degree Sodomy conviction.
- The Board accepted the Voluntary Surrender of Leah M. Woerner’s nursing assistant certificate (000008753CNA) for not meeting certification requirements. She resides in Salem, Ore.
Multnomah County
- The Board revoked the Registered Nurse license of April M. Lavigne (200542380RN) of Troutdale, Ore., for violating the terms and conditions of a Board Order.
- The Board placed the nursing assistant certificate of Samuel W. Wyant (200111251CNA) on probation for two years for conduct unbecoming a nursing assistant. He resides in Portland, Ore.
Washington County
- The Board accepted the Voluntary Surrender of Bethany R. Lamarre’s Registered Nurse license (200341760RN) for conduct related to impaired function. She resides in Tigard, Ore.
Wasco County
- The Board reprimanded the nursing assistant certificate of Naomi M. Enesi (000017808CNA) of The Dalles, Ore., for a conviction, Tampering with Drug Records, and for altering a client record.
Yamhill County
- The Board suspended the nursing assistant certificate of Karen J. Sanderson (200110276CNA) of Sheridan, Ore., for failing to cooperate with the Board during an investigation.
Out of State
- Massachusetts—The Board reprimanded the Registered Nurse license of Susan F. Knight (200642534RN) of Stoughton, Mass., for unauthorized removal of narcotics from the workplace, and practicing nursing while impaired by using drugs.
- Idaho—The Board accepted the Voluntary Surrender of Theresa E. Clark’s Registered Nurse license (200341846RN) for using a controlled substance to the extent that she is unable to practice safely. She resides in Idaho Falls, Idaho.
