Board of Nursing Disciplinary Actions Taken in May 2009
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 25 nurses or nursing assistants during its May 14, 2009 Board Meeting.
 
Clackamas County
  • The Board suspended the nursing assistant certificate of Ann M. Stopper (200411017CNA), of Canby, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Deschutes County
  • The Board suspended the Practical Nurse license of Terry R. McBride (000006268LPN), of Bend, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Douglas County
  • The Board accepted the voluntary surrender of Wendi S. Johnson’s nursing assistant certificate (000008900CNA) for violating the terms and conditions of a Board Order.  She resides in Sutherlin, Ore.
Jackson County
  • The Board accepted the voluntary surrender of Landon R. Georgianna’s nursing assistant certificate (200810896CNA) for using an intoxicant to the extent he is unable to perform CNA duties safely.  He resides in Ashland, Ore.
Klamath County
  • The Board denied the CNA application of Tina Pryor of Klamath Falls, Ore., for misrepresentation during the certification process and failure to answer questions truthfully.
  • The Board revoked the Registered Nurse license of Deborah L. James (200341292RN) of Klamath Falls, Ore., for unauthorized removal of narcotics from the workplace, inaccurate recordkeeping and failure to cooperate during an investigation.
Lane County
  • The Board reprimanded the Practical Nurse license of Andrew B. Smith (200630269LPN) of Veneta, Ore., with conditions for client neglect, failure to take action to preserve client safety and failure to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the voluntary surrender of Margaret R. Jelley’s nursing assistant certificate (200310701CNA) for violating the terms and conditions of a Board Order.  She resides in Springfield, Ore.
Lincoln County
  • The Board accepted the voluntary surrender of Linda S. Bowers’s nursing assistant certificate (200511266CNA) for client neglect, and a physical condition that prevents her from performing CNA duties safely.  She resides in Toledo, Ore.
Linn County
  • The Board reprimanded the nursing assistant certificate of Jessica J. Farler (200711231CNA) of Mill City, Ore., with conditions for assuming duties when competency has not been established or maintained.
Multnomah County
  • The Board suspended the nursing assistant certificate of Jennifer M. Sanchez (200112863CNA), of Portland, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board reprimanded the Practical Nurse license of Denise Howser (200330189LPN) of Portland, Ore., with conditions for falsifying a client record and failure to conform to the essential standards of acceptable nursing practice.
  • The Board suspended the Registered Nurse license of Rosangela Troche (200542582RN), of Portland, Ore., for 60 days with conditions for deceit in the practice of nursing, and failing to conform to the essential standards of acceptable nursing practice.
Polk County
  • The Board placed the certificate of CNA applicant Adam J. King of Dallas, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
Tillamook County
  • The Board accepted the voluntary surrender of Leslie A. Tomlinson’s nursing assistant certificate (200612094CNA) due to a mental condition that prevents her from performing CNA duties safely.  She resides in Tillamook, Ore.
Umatilla County
  • The Board revoked the nursing assistant certificate of George J. Hieter (200811169CNA) of Hermiston, Ore., for using intoxicants to the extent he was unable to perform CNA duties safely.
  • The Board revoked the Registered Nurse license of Tammy L. Paige (096000390RN) of Pendleton, Ore., for practicing nursing when unable to perform due to a mental impairment.
  • The Board placed the certificate of CNA Applicant Kristen N. Awbrey of La Grande, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Wallowa County
  • The Board placed the nursing assistant certificate of Fayelene D. Elson (200812201CNA)of Enterprise, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Washington County
  • The Board reprimanded the Registered Nurse license of Mark S. Logue (097000439RN) of Portland, Ore., with conditions for violating the rights of privacy and confidentiality, and failure to conform to the essential standards of acceptable nursing practice.
  • The Board placed the nursing assistant and medication aide certificates of Kimberly D. Acord (200612530CNA/200820069CMA) of Beaverton, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Yamhill County
  • The Board accepted the voluntary surrender of Genevieve D. Dilkes’ Registered Nurse license (200440340RN) for practicing nursing when unable to perform procedures due to psychological impairment as evidenced by documented deterioration of functioning.  She resides in McMinnville, Ore.
Out of State
  • Washington—The Board suspended the Registered Nurse license of Katherine C. Smith (200341328RN), of Kelso, Wash., indefinitely for failure to cooperate with the Board during an investigation.
  • Washington—The Board revoked the Registered Nurse license of Charles B. Miller (099000302RN) of Vancouver, Wash., for failure to comply with the terms and conditions of the Nurse Monitoring Program.
  • Michigan—The Board accepted the voluntary surrender of Patricia J. Barrios’ Registered Nurse license (098006303RN) for failing to conform to the essential standards of acceptable nursing practice.  She resides in Burlington, Mich.
