The Oregon State Board of Nursing took final disciplinary action against the following 44 nurses or nursing assistants during its November 12, 2008, Board Meeting.
Clackamas County
- The Board reprimanded the Registered Nurse license of Dana S. Hohman (098003194RN) of Estacada, Ore., for failure to follow-through with the plan of care and failure to communicate information regarding client status to members of the healthcare team.
- The Board accepted the Voluntary Surrender of Clare L. King’s Registered Nurse license (084048020RN) for falsifying an agency record, improperly delegating tasks of nursing care and failure to conform to the essential standards of acceptable nursing practice. She resides in Wilsonville, Ore.
- The Board accepted the Voluntary Surrender of Michael J. McNett’s Registered Nurse license and Certified Registered Nurse Anesthetist certificate (076036895RN/076036895CRNA) for practicing nursing while impaired and unauthorized removal of narcotics from the workplace. He resides in Gladstone, Ore.
Columbia County
- The Board suspended the nursing assistant and medication aide certificates of Holly M. Zeuthen (200411088CNA/200520140CMA), of Saint Helens, Ore., for failure to cooperate with the Board during an investigation.
- The Board accepted the Voluntary Surrender of Rachael C. Harris’ Registered Nurse license (094000546RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program, inaccurate recordkeeping, and failure to conform to the essential standards of acceptable nursing practice. She resides in Warren, Ore.
Coos County
- The Board denied the nursing assistant application of Sarah E. Schroeder of Coos Bay, Ore., for failure to cooperate with the Board during an investigation.
- The Board placed the nursing assistant certificate of Nicholas H. Reday (200711635CNA) of Coos Bay, Ore., on probation for two years for using a controlled substance to the extent he was unable to perform CNA duties safely.
Deschutes County
- The Board reprimanded the Registered Nurse license of Dorothy J. Andersen (093006573RN) of Redmond, Ore., for improperly delegating tasks of nursing care to unlicensed persons.
Douglas County
- The Board denied the nursing assistant application of Megan E. Olson of Roseburg, Ore., for performing acts beyond her authorized duties and failure to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Donna J. Nelson (000044702CNA), of Sutherlin, Ore., for failure to cooperate with the Board during an investigation.
- The Board accepted the Voluntary Surrender of Shirley J. Steller’s nursing assistant certificate (200010615CNA) for failure to respect client dignity and rights. She resides in Roseburg, Ore.
Harney County
- The Board suspended the Registered Nurse license of Nancy Carpentier (200541037RN), of Burns, Ore., for a minimum of 30 days with conditions for falsifying a client record, performing acts beyond her authorized scope of practice, and failure to conform to the essential standards of acceptable nursing practice.
Jackson County
- The Board revoked the nursing assistant certificate of Robert H. Kato (200611424CNA) of Medford, Ore., for violating the terms and conditions of a Board Order.
- The Board reprimanded the Registered Nurse license of Virginia L. Rondel (077009921RN) of Medford, Ore., for improperly delegating tasks of nursing care to unlicensed persons and failing to take action to promote patient safety.
- The Board accepted the Voluntary Surrender of Michelle M. Normand’s nursing assistant certificate (000036553CNA) for violating the terms and conditions of a Board order. She resides in Medford, Ore.
Jefferson County
- The Board suspended the Registered Nurse license of Margaret E. Hicks (091003077RN) for a minimum of one year with conditions, to be followed by two years of probation, for unauthorized removal of narcotics from the workplace. She resides in Culver, Ore.
Josephine County
- The Board suspended the nursing assistant and medication aide certificates of Traci A. Powell (000027991CNA/200220144CMA), of Grants Pass, Ore., for failure to cooperate with the Board during an investigation.
- The Board reprimanded the nursing assistant certificate of Molly Meador (200812200CNA) of Grants Pass, Ore., for failure to provide documents requested by the Board.
- The Board reprimanded the Registered Nurse license of Amanda B. Newell (200742251RN) of Cave Junction, Ore., for practicing nursing while impaired by mind-altering substances.
Lane County
- The Board denied the nursing assistant application of Jennifer A. White of Eugene, Ore., for using a controlled substance to the extent she was unable to perform CNA duties.
- The Board suspended the nursing assistant certificate of Jonathan G. Sanchez (200710164CNA), of Junction City, Ore., for failure to cooperate with the Board during an investigation.
- The Board revoked the nursing assistant certificate of Donna M. Miller (200410677CNA) of Cottage Grove, Ore., for violating the terms and conditions of a Board Order.
- The Board revoked the nursing assistant certificate of Teresa L. McCabe (000036501CNA) of Springfield, Ore., for using intoxicants to the extent she is unable to perform CNA duties safely.
- The Board reprimanded the Practical Nurse license of Renay A. Garoutte (084061450LPN) of Springfield, Ore., for failure to follow through with the plan of care and assigning persons to perform functions which are beyond their scope of duties.
- The Board accepted the Voluntary Surrender of Jeanette A. Jacobson’s Registered Nurse license (200440644RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Junction City, Ore.
Lincoln County
- The Board placed the Registered Nurse license of Jennifer A. Hiner (200241724RN) of Newport, Ore., on probation for two years for inaccurate and incomplete recordkeeping, and failure to conform to the essential standards of acceptable nursing practice.
Marion County
- The Board suspended the Practical Nurse license (200230204LPN) of Bruce E. Boyd of Salem, Ore., for 30-days to be followed by a two-year probation for client neglect, failure to take action to preserve client safety and failure to conform to the essential standards of acceptable nursing practice.
- The Board suspended the Nurse Midwife Nurse Practitioner certificate and Registered Nurse license of Patricia M. Sherman (095000158N5/095000158RN) of Stayton, Ore., for 60 days with conditions, for failure to take action to preserve client safety and failure to modify the plan of care as needed.
- The Board accepted the Voluntary Surrender of William L. Owsley’s Registered Nurse license (083040830RN) for engaging in unacceptable behavior toward a client and failure to conform to the essential standards of acceptable nursing practice. He resides in Stayton, Ore.
Multnomah County
- The Board suspended the Practical Nurse license of Patricia F. Ward (095005024LPN), of Portland, Ore., for failure to cooperate with the Board during an investigation.
- The Board revoked the nursing assistant certificate of Nicole S. Eubanks (200311786CNA) of Fairview, Ore., due to a conviction for Attempted Assault 1.
Umatilla County
- The Board suspended the nursing assistant certificate of Katherine K. Balentine (000018596CNA), of Pendleton, Ore., for failure to cooperate with the Board during an investigation.
Washington County
- The Board reinstated the Registered Nurse license (086006901RN) of Darcy L. McDonald of Portland, Ore., and placed it on probation for two-years, upon successful completion of a re-entry program.
- The Board reprimanded the Practical Nurse license of Carla K. McCall (093005184LPN) of Beaverton, Ore., with conditions, and imposed a $500 civil penalty for improperly delegating tasks of nursing care to unlicensed persons, and performing acts beyond her authorized scope of practice.
- The Board placed the nursing assistant certificate of Lisa B. Chandler (200812578CNA) of Beaverton, Ore., on probation for two years for using controlled substances in a manner injurious to self or others.
- The Board suspended the Registered Nurse license of Jessica A. Scharf (200540345RN) for a minimum of 30 days with conditions, to be followed by two years of probation, for practicing nursing when impaired, deceit in the practice of nursing, and use of a controlled substance to the extent she was unable to practice nursing safely. She resides in Portland, Ore.
Yamhill County
- The Board revoked the nursing assistant certificate of Karla M. Ellis (200110732CNA) of McMinnville, Ore., for violating the terms and conditions of a Board Order.
- The Board reprimanded the Practical Nurse license of Yvonne M. Stout (096003008LPN) of Dayton, Ore., for inaccurate and incomplete recordkeeping, and failure to conform to the essential standards of acceptable nursing practice.
- The Board suspended the nursing assistant certificate of Tamara L. Gartner (200212155CNA) of Newberg, Ore., for two years for client abuse and for engaging in other unacceptable behavior toward a client.
Out of State
- California— The Board revoked the nursing assistant certificate of Julie M. Byers (200411472CNA) of Chico, Calif., for violating the terms and conditions of a Board Order.
- California—The Board reprimanded the Registered Nurse license of Dorothy F. Combs (200542491RN) of Fort Bragg, Calif., for failure to implement the plan of care.
- Washington— The Board reprimanded the Registered Nurse license of Teresa M. Altman (080045082RN) of Vancouver, Wash., for client abuse, client neglect, incomplete recordkeeping and failure to conform to the essential standards of acceptable nursing practice.
- Colorado— The Board reprimanded the Registered Nurse license of Barbara A. Gentner (096006895RN) of Loveland, Colo., with conditions, for administering prescription drugs without authorization, incomplete recordkeeping, and failure to conform to the essential standards of acceptable nursing practice.
- Washington—The Board accepted the Voluntary Surrender of Susan F. Solbach’s Registered Nurse license (085080618RN) for practicing nursing while impaired. She resides in Vancouver, Wash.
