The Oregon State Board of Nursing (OSBN) took disciplinary action against the following 16 nurses or nursing assistants during its October 18, 2007, Board Meeting.
Benton County
- The Board revoked the nursing assistant and medication aide certificates of Harold R. Miller (200111184CNA/200220127CMA) of Corvallis, Ore., for violating the terms of a Board Order.
Clackamas County
- The Board revoked the Registered Nurse license of Theresa L. Saban (091000178RN) of Milwaukie, Ore., for using intoxicants to the extent that she is unable to practice nursing safely.
Douglas County
- The Board revoked the nursing assistant certificate of Leonard Stallworth (200210990CNA) of Roseburg, Ore., for violating the terms of Board Order and using intoxicants to the extent that he cannot perform the duties of a nursing assistant safely.
Jackson County
- The Board accepted the voluntary surrender of Julie A. Hill’s Registered Nurse license (200241447RN) for using controlled substances to the extent that she is unable to practice nursing safely. Ms. Hill resides in Central Point, Ore.
Josephine County
- The Board denied the nursing assistant application of Jennifer Lee Bigelow of Grants Pass, Ore., for using intoxicants and controlled substances to the extent that she is unable to perform the duties of a nursing assistant safely.
Lane County
- The Board revoked the Practical Nurse license of Charolotte L. Stimson (200530042LPN) of Eugene, Ore., for violating the terms of a Board Order.
- The Board accepted the voluntary surrender of the Hugo Okonogi’s Registered Nurse license (200340540RN) for practicing nursing while impaired by the use of controlled substances. Mr. Okonogi resides in Eugene, Ore.
Lincoln County
- The Board suspended the Registered Nurse license of Mary E. Miller (098006811RN) of Newport, Ore., for failure to cooperate with the Board during the course of an investigation.
Marion County
- The Board suspended the nursing assistant certificate of Gwenn A. Keuscher (200512611CNA) of Salem, Ore., for failure to cooperate with the Board during the course of an investigation.
- The Board accepted the voluntary surrender of Angela G. Christian’s nursing assistant certificate (000033848CNA) for obtaining controlled substances without a valid prescription. Ms. Christian resides in Salem, Ore.
Multnomah County
- The Board suspended the nursing assistant certificate of Claudia G. Sarver (000035612CNA) of Gresham, Ore., for failure to cooperate with the Board during the course of an investigation.
- The Board accepted the voluntary surrender of Helena E. Gilbert’s Registered Nurse license (200441388RN) for using intoxicants and controlled substances to the extent that she is unable to practice nursing safely. Ms. Gilbert resides in Portland, Ore.
Umatilla County
- The Board revoked the nursing assistant certificate of Nickolas E. Addington (200410719CNA) of Milton Freewater, Ore., for exploiting the client relationship for personal gain.
Wasco County
- The Board accepted the voluntary surrender of Charles M. Herndon’s Registered Nurse license (099006852RN) for using intoxicants to the extent that he is unable to practice nursing safely. Mr. Herndon resides in The Dalles, Ore.
Washington County
- The Board suspended the nursing assistant certificate of Lilian P. Leiva (200311902CNA) of Beaverton, Ore., for 60 days for committing fraud during the licensure process.
Out-of-State
- Washington—The Board placed the Registered Nurse license of Randy S. Buchan (087000446RN) on indefinite probation for failure to act to preserve patient safety, patient neglect and incomplete recordkeeping. Mr. Buchan resides in Vancouver, WA.
