Disciplinary Actions Taken in October 2008
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 35 nurses or nursing assistants during its October 16, 2008, Board Meeting.
 
Benton County
  • The Board revoked the Practical Nurse license of Deborah A. Dereszynski (200630085LPN) of Corvallis, Ore., for fraud during the licensure process and failure to answer questions truthfully.
  • The Board accepted the voluntary surrender of Stuart C. Moore’s Registered Nurse license (097006123RN) for using a controlled substance to the extent he was unable to practice safely, engaging in sexual misconduct, and client abuse.  He resides in Corvallis, Ore.
Clackamas County
  • The Board revoked the nursing assistant certificate of Joshua R. Kell (000034642CNA) of Clackamas, Ore., for violating the terms and conditions of a Board Order.
  • The Board reprimanded the Registered Nurse license of Christa R. Hallowell (200341535RN) of Oregon City, Ore., for failure to answer questions truthfully and misrepresentation during the licensure process.
  • The Board accepted the Voluntary Surrender of Briana D. Bell’s nursing assistant certificate (200511482CNA) for using a controlled substance to the extent she was unable to conduct CNA duties safely and unauthorized possession of controlled drugs.  She resides in Oregon City, Ore.
Columbia County
  • The Board revoked the Pediatric Nurse Practitioner certificate and Registered Nurse license of Connie M. Davis (000038358N2/000038358RN) of Westport, Ore., for performing acts beyond her authorized scope, prescribing medications to clients who were not within her scope of practice, and failing to answer questions truthfully.
Crook County
Jackson County
  • The Board suspended the nursing assistant certificate of Liberty S. Garachico (200611594CNA), of Medford, Ore., for failure to cooperate with the Board during an investigation.
  • The Board suspended the Registered Nurse license of Barbara J. Sonday (200441348RN) for 30-days, to be followed by probation for two years, for failure to answer questions truthfully, inaccurate and incomplete recordkeeping, and falsification of a client record.  She resides in Central Point, Ore.
Josephine County
  • The Board revoked the nursing assistant certificate of Megan M. Kelly (200212916CNA) of Grants Pass, Ore., for violating the terms and conditions of a Board Order.
  • The Board accepted the voluntary surrender of Debra S. Campbell’s nursing assistant certificate (000013987CNA) for client abuse.  She resides in Grants Pass, Ore.
  • The Board accepted the Voluntary Surrender of Cindy C. Gilliland’s Registered Nurse license (200040611RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Grants Pass, Ore.
Lake County
  • The Board suspended the nursing assistant certificate of Tempie L. Crandall (200211346CNA), of Lakeview, Ore., for failure to cooperate with the Board during an investigation.
Lane County
  • The Board denied the medication aide application of Maximo F. Ledesma, of Springfield, Ore., due to a conviction for Grand Theft Property, and fraud during the licensure process.
Malheur County
  •  The Board suspended the nursing assistant certificate of Melissa N. Erickson (200511972CNA), of Vale, Ore., for failure to cooperate with the Board during an investigation.
Marion County
  • The Board revoked the nursing assistant certificate of Jacqueline D. Carter (000041807CNA) of Salem, Ore., due to convictions for Criminal Mistreatment, Possession of a Controlled Substance and Theft.
  • The Board suspended the Registered Nurse license of Anne B. Christofferson (081001037RN) for two years, to be followed by one year of probation, for failing to take action to preserve client safety, client neglect, failure to communicate client information to members of the health care team, and failure to conform to the essential standards of acceptable nursing practice.  She resides in Salem, Ore.
Multnomah County
  • The Board suspended the nursing assistant certificate of Julie A. Clausen (200511774CNA), of Portland, Ore., for failure to cooperate with the Board during an investigation.
  • The Board suspended the nursing assistant certificate of Shannele M. Garner (200213142CNA), of Gresham, Ore., for failure to cooperate with the Board during an investigation.
  • The Board suspended the nursing assistant certificate of Dora Myers (000043100CNA), of Portland, Ore., for failure to cooperate with the Board during an investigation.
  • The Board reprimanded the nursing assistant certificate of Twila L. Harrington (200410638CNA), of Portland, Ore., for client abuse and failure to respect client rights and dignity.
  • The Board revoked the medication aide and nursing assistant certificates of Karey S. Lostaglia (200411081CNA/200520001CMA) of Portland, Ore., for removing drugs from the workplace without authorization, inaccurate and incomplete recordkeeping, and failure to administer medications as ordered.
  • The Board revoked the  nursing assistant certificate of Theresa K. Smith (200213054CNA) of Portland, Ore., for use of a controlled substance to the extent she was unable to perform CNA duties safely, client abuse, exploiting the client relationship for personal gain, unauthorized possession of controlled drugs, and failure to cooperate with the Board during an investigation.
  • The Board denied a request for reinstatement of Cassandra L. Yeoman’s Registered Nurse license (094006329RN) due to a Bank Robbery conviction and using alcohol and illegal drugs to the extent that she is unable to practice nursing safely.  She resides in Gresham, Ore.
  • The Board accepted the Voluntary Surrender of Suzanne K. Ruddell’s Registered Nurse license (200240120RN) due to a Criminal Mistreatment conviction.  She resides in Troutdale, Ore.
Polk County
Sherman County
  • The Board revoked the nursing assistant certificate of Jamie A. McCurdy (200511623CNA) of Wasco, Ore., for being convicted of Unlawful Delivery of Methamphetamine and Unlawful Possession of Methamphetamine.
Umatilla County
  • The Board accepted the Voluntary Surrender of Katherine R. Molloy’s Registered Nurse license (200541420RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Hermiston, Ore.
Washington County
  • The Board suspended the nursing assistant certificate of Deborah D. Wilson (200411095CNA), of Beaverton, Ore., for failure to cooperate with the Board during an investigation.
  • The Board accepted the Voluntary Surrender of Olivia M. Wahl’s Registered Nurse license (200540612RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Cornelius, Ore.
Yamhill County
  • The Board placed the nursing assistant certificate of Jessica Y. Cluver (000027519CNA) on probation for two years for using an intoxicating liquor to the extent that she was unable to perform CNA duties safely.  She resides in McMinnville, Ore.
  • The Board accepted the Voluntary Surrender of Mary A. Lasslett’s Registered Nurse license (095000182RN) for using a controlled substance to the extent that she is unable to practice safely, and violating the terms and conditions of Board Order.  She resides in Amity, Ore.
Out of State
  • Louisiana—The Board suspended the Registered Nurse license of Sharon A. Fontenot (200742950RN), of Sulphur, La., for failure to cooperate with the Board during an investigation.
  • Hawaii—The Board revoked the Practical Nurse license of Mary L. Taylor (093003030LPN) of Naalehu, Hawaii, for the unauthorized removal of property from the workplace, using the client-nurse relationship to exploit the client for personal gain, violating client confidentiality rights, and unauthorized removal of client information from the workplace.
  • Washington—The Board revoked the Registered Nurse license of Glenda J. Baxter (080045320RN) of Ridgefield, Wash., for failure to take action to preserve client safety, client neglect, assigning to a person functions beyond their scope of duties, and failure to conform to the essential standards of acceptable nursing process.
