The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 35 nurses or nursing assistants during its October 14, 2009 Board Meeting.
Benton County
- The Board suspended the Registered Nurse license of Suzanne J. Brown (080045381RN), of Corvallis, Ore., for three years with conditions, to be followed by probation for one year, for failing to comply with the terms and conditions of the Nurse Monitoring Program.
Clackamas County
- The Board placed the Nursing Assistant certificate of Hans J. Detter (200912913CNA) of Portland, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
- The Board reprimanded the Registered Nurse license of Yoo Shin Kim (098006816RN), of Damascus, Ore., with conditions, for failing to take action to promote client safety, client neglect and failing to conform to the essential standards of acceptable nursing practice.
- The Board suspended the Nursing Assistant and Medication Aide certificates of Ginger L. Hull (000027611CNA/000027611CMA), of Sandy, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Clatsop County
- The Board accepted the voluntary surrender of Merrie A. Flavin’s Nursing Assistant certificate (200811540CNA) for using the client relationship for personal gain, leaving a CNA assignment without properly notifying a supervisor, and removing drugs from the workplace without authorization. She resides in Astoria, Ore.
Coos County
- The Board accepted the voluntary surrender of Ruth O. Ford’s Nursing Assistant certificate (200410971CNA) for failing to answer licensure questions truthfully. She resides in North Bend, Ore.
- The Board reprimanded the Practical Nurse license of Susanne Ellenberger (200630014LPN), of Coquille, Ore., with conditions, for inaccurate and incomplete recordkeeping and failing to conform to the essential standards of acceptable nursing practice.
- The Board suspended the Nursing Assistant certificate of Brenda K. Green (200810406CNA), of Myrtle Point, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Deschutes County
- The Board placed the Nursing Assistant certificate of Shannon K. Webb (200110311CNA) of Redmond, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board accepted the voluntary surrender of Dan. A. Ankrom’s Registered Nurse license (091006938RN) for the unauthorized removal of drugs from the workplace, failing to dispense medications, and inaccurate recordkeeping. He resides in Bend, Ore.
Douglas County
- The Board accepted the voluntary surrender of Cindy M. Cox’s Nursing Assistant certificate (200712454CNA) for using the client relationship for personal gain. She resides in Glide, Ore.
Jackson County
- The Board accepted the voluntary surrender of Arleta F. Walker’s Nursing Assistant certificate (000009547CNA) for failing to respect client rights, and performing acts beyond her authorized duties. She resides in White City, Ore.
- The Board accepted the voluntary surrender of Shannon R. Stromberg’s Registered Nurse license (200541663RN) for practicing nursing while impaired, and unauthorized removal of drugs from the workplace. She resides in White City, Ore.
Josephine County
- The Board suspended the Nursing Assistant certificate of Jill A. Colin (200710223CNA), of Grants Pass, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Lane County
- The Board suspended the Nursing Assistant and Medication Aide certificates of Desteny M. Rogers (000035548CNA/200620091CMA), of Cottage Grove, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Linn County
- The Board approved an emergency suspension of Trinity H. Vidal Hernandez’s Nursing Assistant certificate (200811530CNA) indefinitely due to a mental condition that prevents her from performing CNA duties safely. She resides in Albany, Ore.
- The Board approved an emergency suspension of Rhonda L. Ackley’s Registered Nurse license (200241558RN) indefinitely for using intoxicants to the extent she was unable to practice nursing safely, failing to complete a nursing assignment without notifying appropriate personnel, and unauthorized removal of drugs from the workplace. She resides in Albany, Ore.
Marion County
- The Board denied the CNA application of Renee S. Bostwick of Salem, Ore., for Criminal Mistreatment conviction.
- The Board placed the Registered Nurse license of Virginia L. Tan (200942916RN) of Keizer, Ore., on probation for one year due to a Theft conviction.
- The Board placed the Nursing Assistant certificate of Daniel S. Ruby (200610117CNA) of Aumsville, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
- The Board placed the Nursing Assistant certificate of Jesus E. Espinoza-Gallegos (200512189CNA) of Salem, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
Multnomah County
- The Board accepted the voluntary surrender of Carrol A. Staley’s Practical Nurse license (081000355LPN) for failing to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Portland, Ore.
- The Board accepted the voluntary surrender of Kelly L. Brady’s Registered Nurse license (089006523RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Portland, Ore.
- The Board placed the Nursing Assistant certificate of applicant Deborah E. Bell of Portland, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board suspended the Nursing Assistant certificate of Jade D. Lilienthal (200910582CNA), of Portland, Ore., indefinitely for failing to cooperate with the Board during an investigation.
- The Board denied the CNA application of Clifford Bechard of Portland, Ore., for violating the terms and conditions of a Board Order, and failing to answer questions truthfully.
Polk County
- The Board placed the Nursing Assistant certificate of Donna A. Shamblin (200812730CNA) of Salem, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board suspended the Nursing Assistant certificate of Nancy A. Saludo (200612507CNA), of Dallas, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Umatilla County
- The Board revoked the Registered Nurse license of Fawn M. Hasty (089000185RN), of Athena, Ore., for inaccurate and incomplete recordkeeping, failing to dispense medications, and failing to follow through with the plan of care.
Washington County
- The Board placed the Nursing Assistant certificate of Valerie Perez (200312228CNA) of Beaverton, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Yamhill County
Out of State
- Washington—The Board placed the Nursing Assistant certificate of Applicant John P. Moore of Washougal, Wash., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
- Washington—The Board accepted the voluntary surrender of Clare E. Blodgett’s Registered Nurse license (200742896RN) for using intoxicants to the extent she is unable to practice nursing safely. She resides in Everett, Wash.
- California—The Board accepted the voluntary surrender of Jeff D. Holland’s Registered Nurse license (200542591RN) for inaccurate and incomplete recordkeeping, practicing nursing while impaired, and unauthorized removal of drugs from the workplace. He resides in Citrus Heights, Calif.
- Florida—The Board denied the RN application of Joseph R. Cassamajor of Palm Beach, Fla., for failing to meet educational standards and misrepresentation during the licensure process.
