Disciplinary Actions Taken in September 2007
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 53 nurses or nursing assistants during its September 19 disciplinary Board Meeting.
 
Benton County
  • The Board accepted the voluntary surrender of Theodore W. Schroeder’s Registered Nurse license (098000165RN) for using intoxicants to the extent that he is unable to practice nursing safely.  Mr. Schroeder resides in Philomath, OR.
Clackamas County
  • The Board reprimanded the Registered Nurse license of Leslie C. Kunz (200140713RN) of Milwaukie, OR, for failure to failure to answer questions truthfully.
  • The Board accepted the Interim Order by Consent signed by Judith R. Richardson (000027095RN) of Oregon City, OR.  She is prohibited from practicing as a Registered Nurse in any capacity.
  • The Board accepted the voluntary surrender of Brandy L. Granziero’s practical nurse license (099005058LPN) for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Granziero resides in Wilsonville, OR.
Coos County
Crook County
  • The Board suspended the nursing assistant certificate of Krystal Anne Borden (200310308CNA) of Prineville, OR, for failure to cooperate with the Board during the course of an investigation.
Deschutes County
  • The Board accepted the voluntary surrender of Julie A. Miller’s Registered Nurse license (099000459RN) for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Miller resides in Bend, OR.
  • The Board accepted the voluntary surrender of Jennifer L. Schram’s nursing assistant certificate (200610593CNA) for falsifying agency records.  Ms. Schram resides in Bend, OR.
  • The Board accepted the Interim Order by Consent signed by Annett J. Coats (082010459RN) of Bend, OR.  She is prohibited from performing the duties of a Registered Nurse in any capacity.
Douglas County
  • The Board accepted the voluntary surrender of Nancy V. Ferguson’s Registered Nurse license (000006573RN) for practicing nursing when unfit due to a physical impairment.  Ms. Ferguson resides in Roseburg, OR.
  • The Board accepted the voluntary surrender of Cecelia E. Steele’s Registered Nurse license (089000245RN) for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Steele resides in Glendale, OR.
Hood River County
  • The Board reprimanded the Registered Nurse license of Nancy L. Schwartz (097003116RN) of Mount Hood Parkdale, OR, for failure to modify the plan of care as needed based on nursing assessment and incomplete recordkeeping.
Jackson County
  • The Board accepted the voluntary surrender of Robbie W. Grizzle’s Registered Nurse license (085079827RN) for using intoxicants to the extent that he is unable to practice nursing safely.  Mr. Grizzle resides in Medford, OR.
  • The Board accepted the voluntary surrender of Nicole Robustelli’s Registered Nurse license (200141391RN) for practicing nursing while impaired by use of drugs and for an Identify Theft conviction.  Ms. Robustelli resides in Medford, OR.
  • The Board accepted the voluntary surrender of Elizabeth L. Blaney’s Registered Nurse license (085081711RN) for client abuse and neglect.  Ms. Blaney resides in Medford, OR.
  • The Board accepted the voluntary surrender of Gary D. Pritchett’s practical nurse license (089005056LPN) for using intoxicants to the extent that he is unable to practice nursing safely.  Mr. Pritchett resides in Medford, OR.
Josephine County
  • The Board accepted the voluntary surrender of Cynthia R. Rapant’s Registered Nurse license (096003194RN) for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Rapant resides in Grants Pass, OR.
Lane County
  • The Board suspended the nursing assistant certificate of Linda C. Szymanski (000021799CNA) of Eugene, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board reprimanded the Registered Nurse license of Suzanne M. Richards (078039863RN) of Veneta, OR, for using profane language in the presence of a patient.
  • The Board reprimanded the Family Nurse Practitioner certificate of Mary A. Beedle (200150004NP) of Eugene, OR, for failure to comply with rules for prescribing, administering and distributing drugs.
  • The Board accepted the Interim Order by Consent signed by Sharon L. Carter (087000125RN) of Florence, OR.  She is prohibited from performing the duties of a Registered Nurse in any capacity.
  • The Board accepted the Interim Order by Consent signed by Bonnie K. Curran (200230337LPN) of Pleasant Hill, OR.  She is prohibited from performing the duties of a Licensed Practical Nurse in any capacity.
  • The Board placed the Registered Nurse license of Glenda J. Spencer (092003284RN) of Springfield, OR, on probation for 12-months for inaccurate and incomplete recordkeeping.
Linn County
  • The Board suspended the nursing assistant certificate of Linda L. Ivie (200611538CNA) of Sweet Home, OR, for 30-days for failure to respect client rights and engaging in derogatory and profane language in the presence of a client.
Malheur County
  • The Board accepted the voluntary surrender of Allison Wharton’s nursing assistant certificate (200511838CNA) for exploiting the client relationship for personal gain.  Ms. Wharton resides in Ontario, OR.
Marion County
  • The Board accepted the voluntary surrender of Jessica E. Fowler’s nursing assistant and medication aide certificates (200112160CNA/200220175CMA) for using intoxicants to the extent that she is unable to perform CNA/CMA duties safely.  Ms. Fowler resides in Salem, OR.
Multnomah County
  • The Board suspended the nursing assistant certificate of Deborah J. Gregory (000035423CNA) of Portland, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board suspended the nursing assistant certificate of Libra R. Gauger (000025148CNA) of Portland, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board suspended the nursing assistant certificate of Melissa F. Acevedo (200612283CNA) for 30-days for fraud during the licensure process.
  • The Board reprimanded the practical nurse license of Shannon R. McCartor (080012437LPN) of Portland, OR, for failure to take action to preserve client safety and failure to communicate the client’s status to other members of the healthcare team.
  • The Board accepted the Interim Order by Consent signed by Laura M. Manion (200313248CNA) of Portland, OR.  She is prohibited from performing the duties of a nursing assistant or functioning as a caregiver in any setting.
  • The Board accepted the Interim Order by Consent signed by April M. Lavigne (200542380RN) of Troutdale, OR.  She is prohibited from performing the duties of a Registered Nurse in any capacity.
  • The Board suspended the nursing assistant certificate of Lisa M. Moore (200311125CNA) of Portland, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board suspended the nursing assistant certificate of Jessica L. Turner (200610830CNA) of Gresham, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board suspended the nursing assistant certificate of Brenda K. Ellsworth (200610757CNA) of Gresham, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board accepted the Interim Order by Consent signed by Emily C. Arnold (200340899RN) of Gresham, OR.  She is prohibited from performing the duties of a Registered Nurse in any capacity.
  • The Board accepted the Interim Order by Consent signed by Kelsey R. Meyer (200313637CNA) of Portland, OR.  She is prohibited from performing the duties of a Certified Nursing Assistant in any capacity.
  • The Board revoked Valerie M. Lutz’s nursing assistant certificate (000010215CNA) for failure to answer questions truthfully and convictions in 2006 for Possession of a Controlled Substance and Endangering the Welfare of a Minor.  Ms. Lutz resides in Portland, OR.
  • The Board denied the Certified Nursing Assistant application of Vickie D. Turner of Portland, OR, for misrepresentation during the certification process.
Polk County
  • The Board accepted the Interim Order by Consent signed by John Clay (200441562RN) of Salem, OR.  He is prohibited from performing the duties of a Registered Nurse in any capacity.
Umatilla County
  • The Board placed the Registered Nurse license of Michelle L. Wallace (090000436RN) on probation for two years for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Wallace resides in Pendleton, OR.
Wallowa County
  • The Board accepted the Interim Order by Consent signed by Tanya J. Gorbett (200112073CNA) of Enterprise, OR.  She is prohibited from performing the duties of a nursing assistant or functioning as a caregiver in any setting.
Wasco County
  • The Board accepted the voluntary surrender of Catherine Taylor’s Registered Nurse license (089007595RN) for failure to conform to the essential standards of acceptable and prevailing nursing practice.  Ms. Taylor resides in Mosier, OR.
  • The Board accepted the Interim Order by Consent signed by William W. Reaves (200140965RN) of The Dalles, OR.  He is prohibited from performing the duties of a Registered Nurse in any capacity.
Washington County
Yamhill County
  • The Board suspended the nursing assistant certificate of Melissa S. Rummell (200110275CNA) of McMinnville, OR, for failure to cooperate with the Board during the course of an investigation.
  • The Board reprimanded the nursing assistant certificate of Joeen M. Blakley (200310495CNA) of Amity, OR, for failure to perform nursing assistant duties competently.
  • The Board suspended the nursing assistant certificate of Heather M. Savidge (200111797CNA) of McMinnville, OR, for failure to cooperate with the Board during the course of an investigation.
Out of State
  • California—The Board reprimanded the Registered Nurse license of Bethany H. Reyman (200642390RN) of Crescent City, CA, for failure to respect the dignity of patients and violating confidentiality rights.
  • Colorado— The Board accepted the Interim Order by Consent signed by Susan J. Neeley (098000359RN) of Grand Junction, CO.  She is prohibited from performing the duties of a Registered Nurse in any capacity.
  • Indiana—The Board accepted the voluntary surrender of Leslie A. Fitzgerald’s Registered Nurse license (200740330RN) for using intoxicants to the extent that she is unable to practice nursing safely.  Ms. Fitzgerald resides in Indianapolis, IN.
