|
Here are a few things you should know before beginning:
- If you hold an advanced practice certificate or are a medication aide, please have your continuing education information handy. You will be required to list that information during the renewal process.
- If you hold a nurse practitioner certificate with prescriptive privileges, please have your DEA number handy. You will be required to provide that number.
- You will need to supply your license/certificate number, Social Security number and date of birth to log into the application.
- After you log-in, you will reach your "My Licenses" page, which will display your license(s).
- If you hold more than one license type, you must go through the renewal process for each license. The license you will renew first will display a "Renew" button. Subsequent licenses are listed, but cannot be selected until you finish the first renewal.
- You will be able to verify your mailing address and current employment information at any time (according to the Nurse Practice Act, licensees and certificate-holders must report address changes to the OSBN). However, you will only be able to access the renewal portion of our online service when you are in your 60-day renewal cycle.
- Your online renewal may be paid by credit card, personal check, cashier´s check or money order. If you choose to pay with a check or money order, your application will not be processed until the payment has been received in our office.
- You may not renew online if you answer "yes" to any of the legal/discipline qualifiers listed in the section below.

| Legal/Discipline Qualifiers |

If you answer "yes" to any of the following questions, you may not renew online. You will need to continue your renewal process using our standard paper application forms.
- Do you have a physical, mental or emotional condition that in any way impairs your ability to practice nursing/perform nursing assistant duties with reasonable skill and safety?
- Since the date of your last renewal, have you been arrested, charged with, entered a plea of guilty, no contest, convicted of or been sentenced for any criminal offense either misdemeanor or felony, including driving under the influence, in any state? (The fact that a conviction has been pardoned, expunged (excluding juvenile record expungement), dismissed or that your civil rights have been restored does not mean that you answer this question “NO”; you would answer “YES” and give full details & circumstances of the charge).
- Since the date of your last renewal, have you been found in any proceeding to have violated any state or federal law or rule regulating the practice of a health care profession?
- Have any disciplinary actions been taken (or are any actions pending) against your nursing license/nursing assistant certificate in any state or U.S. jurisdiction?
- Since the date of your last renewal, have you suffered any civil judgment for incompetence, negligence or malpractice concerning the practice of a health care professional?
- Do you use, or have you used in the time since your last renewal, chemical substance(s) in any way, which impairs or limits your ability to practice nursing/perform nursing assistant duties with reasonable skill and safety? “Chemical Substance” includes alcohol and drugs.
- Are you currently engaged in the unlawful use of controlled substance(s)? (Unlawful use of controlled substances means the use of controlled substances obtained illegally (e.g. marijuana, meth, heroin, cocaine) as well as the use of legally obtained controlled substances, not taken in accordance with the directions of a licensed health care provider.)
- Since the date of your last renewal, have you been found in any civil, administrative or criminal proceeding to have possessed, used, prescribed for use or distributed controlled substances or prescription drugs in any way other than for legitimate or therapeutic purposes, diverted controlled substances or prescription drugs, violated any drug law or prescribed controlled substances for yourself?
- Since the date of your last renewal, have you been found in any civil, administrative or criminal proceeding to have committed any act involving dishonesty or corruption?
- Since the date of your last renewal, have you had any certificate, license, registration or other privilege to practice a health care profession denied, revoked, suspended, restricted, reprimanded, censured or placed on probation by a state, federal or foreign authority, or have you ever surrendered such credential to avoid, or in connection with, action by such authority?
- Since the date of your last renewal, have you been investigated (regardless of whether the investigation was substantiated) for any type of abuse in any state?

- I have entered all my log-in information, but still can´t get to the "My Licenses" page. Why?
Your record in our database may be flagged for some reason. Please contact the OSBN office at 971-673-0685 for more information.
- I am not due to renew my license until next year, but I´ve moved and need to update my address. How?
You are able to access the "Personal Information" section of your online file at anytime. Log into the application, then click on "My Information" in the left-hand vertical bar.
- I am a nurse practitioner who graduated from my NP program last year. How do I know how many continuing education credits I need?
When you get to the Continuing Education section of the online renewal process, the application will automatically compute and list how many CEs you need, based on your graduation date.
- Why do you need my Social Security number?
The OSBN is required to collect licensees´ Social Security numbers by state and federal law. Click here for more information.

| Refund Policy-Licensure Fees |

Fees submitted as part of the application process are not refundable, per ORS 678.410(2). Be certain that you intend to complete the process before submitting your payment. If you have any questions regarding your eligibility, please call our Customer Service Center at 971-673-0685.
If an error has been made in your payment, contact OSBN Information: 971-673-0685, or e-mail oregon.bn.info@state.or.us to request a review of your account. Allow 10 working days for the refund to be processed as a credit to your credit card account.
