Board of Nursing Disciplinary Actions Taken in May 2012

Benton County
  • The Board reprimanded the nursing assistant certificate of Angel L. E. Goetzinger (200212570CNA) with conditions for a Criminal Mistreatment 2 conviction. She resides in Corvallis, Ore.
Clackamas County
  • The Board accepted the voluntary surrender of John M. Richards’ Registered Nurse license and Adult Nurse Practitioner certificate (000038332RN/000038332N3) for violating the terms and conditions of the Health Professionals’ Services Program. He resides in Oregon City, Ore.
Coos County
  • The Board suspended Rochella C. Petitt’s nursing assistant certificate (201110615CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in North Bend, Ore.
Douglas County
  • The Board placed Neoma J. Keeler’s Registered Nurse license (200340207RN) on probation for failing to comply with the terms and conditions of the Health Professionals’ Services Program. She resides in Roseburg, Ore.
  • The Board denied the CNA application of Brent T. Nasset, of Glide, Ore., for using intoxicants to the extent injurious to himself or others.

Jackson County
  • The Board accepted the voluntary surrender of Tanya M. Arcouette-Melnikoff’s Registered Nurse license (078010299RN) for violating the terms and conditions of the Health Professionals’ Services Program. She resides in White City, Ore.
  • The Board placed Diana L. Lockwood’s Registered Nurse license (200740540RN) on probation for failing to comply with the terms and conditions of the Health Professionals’ Services Program. She resides in Medford, Ore.

Klamath County
  • The Board placed Cynthia K. Moran’s Registered Nurse license (200540023RN) on probation for unauthorized possession of prescription drugs, using intoxicants to the extent injurious to herself or others, and failing to conform to the essential standards of acceptable nursing practice. She resides in Klamath Falls, Ore.
Lane County
  • The Board revoked the Registered Nurse license of Leaha M. Howard (200040578RN), of Dorena, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
Linn County
  • The Board placed Joy L. Strasser’s Registered Nurse license (092006185RN) on probation for failing to comply with the terms and conditions of the Health Professionals’ Services Program. She resides in Sweet Home, Ore.
Marion County
  • The Board revoked the nursing assistant certificate of Chad J. Skinner (200911685CNA), of Salem, Ore., for using intoxicants to the extent injurious to himself or others, and failing to cooperate with the Board during an investigation.
  • The Board placed Karen A. Rabb’s Registered Nurse license (095003035RN) on probation for using intoxicants to the extent injurious to herself or others, and failing to conform to the essential standards of acceptable nursing practice. She resides in Salem, Ore.
  • The Board denied the LPN application of Steven D. Nelson, of Silverton, Ore., for multiple convictions and failing to answer questions truthfully.

Polk County
  • The Board accepted the voluntary surrender of Phillip M. Huber’s practical nurse license (201130495LPN) for violating the terms and conditions of the Health Professionals’ Services Program. He resides in Salem, Ore.
Washington County
  • The Board accepted the voluntary surrender of Suzette M. Burris’ Registered Nurse license (085075508RN) for violating the terms and conditions of a Board Order. She resides in Hillsboro, Ore.
  • The Board denied the CNA renewal application of Charlene L. Arce (200812101CNA), of Beaverton, Ore., for failing to cooperate with the Board during an investigation and failing to provide requested documents.

Out of State
  • Washington—The Board revoked the Registered Nurse license of Christopher L. Carlson (200842909RN), of Vancouver, Wash., for failing to cooperate with the Board during an investigation.
  • Iowa—The Board revoked the Registered Nurse license of Angela D. Sherwood (201140563RN), of Fairfax, Iowa, for misrepresentation during the licensure process, and failing to cooperate with the Board during an investigation.
  • Kentucky—The Board accepted the voluntary surrender of Ian S. Schwartz’s Registered Nurse license (200140038RN) due to a conviction for Sexual Exploitation of Children. He resides in Louisville, Ky.
