Board of Nursing Disciplinary Actions Taken in November 2011
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 30 nurses or nursing assistants during its November 16, 2011, Board Meeting.

Clackamas County
  • The Board accepted the voluntary surrender of Cathy D. Martin’s Registered Nurse license (097000245RN) for failing to conform to the essential standards of acceptable nursing practice. She resides in Milwaukie, Ore.
  • The Board placed the Registered Nurse license of Neil Larocco (096006633RN) on probation for two years for using intoxicants to the extent injurious to himself or others. He resides in Clackamas, Ore.
  • The Board placed the practical nurse license of Leanna Nolan (083043023LPN) on probation for 24-months for using intoxicants to the extent injurious to herself or others. She resides in West Linn, Ore.
  • The Board suspended Isela Paez Alvarado’s practical nursing license (201030066LPN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in Milwaukie, Ore.

Coos County
  • The Board reprimanded the nursing assistant certificate of Heather M. Williams (200611662CNA) for client neglect and failing to perform CNA duties competently. She resides in Coos Bay, Ore.

Douglas County
  • The Board suspended Anna M. Fladager’s Registered Nurse license (200440846RN) for a minimum of 14 days, for violating the terms and conditions of a Board Order. She resides in Roseburg, Ore.

Grant County
  • The Board placed the nursing assistant certificate of applicant Jill J. Brown on probation for 24-months for using intoxicants to the extent injurious to herself or others. She resides in John Day, Ore.

Hood River County
  • The Board suspended Brittany D. Thornton’s nursing assistant certificate (201012355CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in Hood River, Ore.

Jackson County
  • The Board accepted the voluntary surrender of David M. Davis’s nursing assistant certificate (201011312CNA) for using intoxicants to the extent injurious to himself or others. He resides in Medford, Ore.

Lane County
  • The Board accepted the voluntary surrender of Karen L. Sercus’s Registered Nurse license (094006357RN) for using intoxicants to the extent injurious to herself or others. She resides in Cottage Grove, Ore.
  • The Board accepted the voluntary surrender of Sherry M. Larson’s nursing assistant certificate (000036231CNA) for attempting to obtain unauthorized prescription drugs. She resides in Springfield, Ore.
  • The Board accepted the voluntary surrender of Mae V. Hunt’s Registered Nurse license (092003234RN) for falsifying a client record. She resides in Veneta, Ore.
  • The Board revoked the nursing assistant certificate of Troy G. Monroe (200910407CNA), of Junction City, Ore., for exploiting the client relationship for personal gain.

Lincoln County
  • The Board revoked the nursing assistant certificate of Joana M. R. Underhill (200810462CNA), of South Beach, Ore., due to a felony Sexual Abuse conviction.

Linn County
  • The Board reprimanded the Registered Nurse license of Amy L. Wheeler (201042731RN) for failing to conform to the essential standards of acceptable nursing practice. She resides in Sweet Home, Ore.
  • The Board accepted the voluntary surrender of Debbie L. Rediger’s Registered Nurse license (079011461RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program. She resides in Albany, Ore.
  • The Board reprimanded the nursing assistant certificate of Abbalue Tarvins-Dunworth (201011630CNA) with conditions for client neglect and failing to perform CNA duties competently. She resides in Albany, Ore.

Marion County
  • The Board accepted the voluntary surrender of Duane M. Jenner’s Registered Nurse license (086000309RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program. He resides in Salem, Ore.
  • The Board revoked the Registered Nurse license of Shirley A. Miller (200241947RN), of Salem, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program, and failing to cooperate with the Board during an investigation.

Multnomah County
  • The Board accepted the voluntary surrender of Jerie E. Parish’s Registered Nurse license (200141850RN) for using intoxicants to the extent injurious to herself or others. She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Jonathan B. Roberson (200712513CNA), of Portland, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board reprimanded the nursing assistant certificate of Michael L. Myers (200310964CNA) with conditions for engaging in unacceptable behavior toward a client. He resides in Gresham, Ore.
  • The Board placed the nursing assistant certificate of Kimmie R. Melgarejo (201010949CNA) on probation for 24-months. She resides in Portland, Ore.
  • The Board accepted the voluntary withdrawal of Cynthia M. Dove’s application to reactivate her Registered Nurse license (086000079RN) for unauthorized removal of narcotics from the workplace, and failing to conform to the essential standards of acceptable nursing practice. She resides in Portland, Ore.

Umatilla County
  • The Board reprimanded the Registered Nurse license of Marlene I. Meadows (200940077RN) with conditions for failing to conform to the essential standards of acceptable nursing practice. She resides in Athena, Ore.

Wasco County
  • The Board suspended Christopher N. Rice’s Registered Nurse license (081001590RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. He resides in The Dalles, Ore.

Washington County
  • The Board placed the nursing assistant certificate of Angela L. Blake (200110467CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others. She resides in Portland, Ore.

Yamhill County
  • The Board revoked the nursing assistant certificate of Kelly J. Brannon (200512587CNA), of Dayton, Ore., for using intoxicants to the extent injurious to herself or others, and failing to provide documents requested by the Board.

Out of State
  • Washington—The Board revoked the nursing assistant certificate of Victoria M. Brown (000032429CNA), of Longview, Wash., for client abuse and neglect, and failing to perform CNA duties competently.
  • California—The Board accepted the voluntary surrender of Victoria J. Lovio’s Registered Nurse license (200943303RN) for unauthorized removal of narcotics and supplies from the workplace. She resides in Mount Shasta, Calif.
