| DNA Test Kits (CODIS Kits) |
|
DNA testing kits (also known as CODIS kits or Convicted Offender kits) can be ordered direct from the Portland Metro Laboratory.
Kit orders or questions regarding the collection process should be directed to Heidi Stetson at 971-673-8267 or heidi.stetson@state.or.us. Be prepared to give your name, phone number, agency name and address where the kits should be sent to.

| SAFE (Sexual Assault) Kits |
|
SAFE (Sexual Assault Forensic Evidence) kits can be ordered direct from the manufacturer, Portland Habilitation Center (PHC). They are a non-profit rehabilitation facility.
Requests for kits should be emailed to fulfillment@phcnw.com. When ordering, please indicate how many boxes are needed. There are 15 kits per box, and boxes will not be split up for partial orders. Please limit your order to less than three boxes at a time.
Be sure to include the shipping address, phone number and contact name when the order is placed.
Questions pertaining to SAFE kits should still be directed to your local forensic laboratory.

| Urine Sample Kits |
|
Urine sample kits are provided by the Oregon State Police for the collection of urine samples under Oregon's Implied Consent Law. They are to be used when subjects are arrested on DUII charges.
Requests for urine sample kits should be made electronically to Sr. Trooper Michael Iwai at michael. iwai@state.or.us. Please include your name, phone number, agency name, address where the kits should be mailed to, and quantity requested.

| Blood Collection Kits |
|
Blood collection kits should be ordered via fax using the OSP Stockroom Order Form. Only request enough items to last your agency for approximately three months. Please remember to include your shipping information at the bottom of the form.
Fax requests to Oregon State Police/Supply at (503) 463-8219. PHONE REQUESTS ARE DISCOURAGED. Orders will be filled and shipped promptly unless items are backordered.
