Criminal Reports (PDF) - Select this form to request investigations of crimes.
- This form was designed to help the GENERAL PUBLIC appropriately request access to criminal police records. Please provide as much information as known, if not known leave area on form blank. Please note that a reason for the request and a signature are required.
- If you are a representative of a Government Agency, an Attorney or other Business please submit your written request on letterhead.
- If you are a representative of an Insurance Company please use your company’s standard request process and form.
Media Requests (PDF) -Select this form if you are a media representative seeking to obtain a police report for a media purpose.
- This form is designed to facilitate the prompt response to a reporters request for records a faxed copy of this form is all that is necessary to initiate a request
- Requests by the media for criminal police reports should be made to General Headquarters
