Public Records Request Forms
Please be advised that to actually provide any requested POLICE REPORT a written request with signature and reason for request accompanied by the appropriate payment must be received.  See instructions attached with the form you choose to use below.

Criminal Reports (PDF) - Select this form to request investigations of crimes. 
  • This form was designed to help the GENERAL PUBLIC appropriately request access to criminal police records.  Please provide as much information as known, if not known leave area on form blank.  Please note that a reason for the request and a signature are required.
  • If you are a representative of a Government Agency, an Attorney or other Business please submit your written request on letterhead.
  • If you are a representative of an Insurance Company please use your company’s standard request process and form.
 
Media Requests (PDF) -Select this form if you are a media representative seeking to obtain a police report for a media purpose.
  • This form is designed to facilitate the prompt response to a reporters request for records a faxed copy of this form is all that is necessary to initiate a request
  • Requests by the media for criminal police reports should be made to General Headquarters
  • This form must be printed, signed and either faxed or mailed to the Oregon State Police Criminal Division
 
Traffic Reports (PDF) - Select this form to request traffic related reports involving driving charges and DUII.
  • This form was designed to facilitate requests from insurance companies and attorneys seeking traffic related reports involving driving charges and DUII.
  • The GENERAL PUBLIC may also use this form to make their written request.
