HazMat Team Responses
This search will list all of the incidents that have occurred which required a HazMat Team Response.
 
Incident Count
This search sums the number of incidents between the start date and end date entered.
 
Incidents Between Dates
This search will list all of the incidents that occurred between the start date and end date entered.
 
Incidents by Chemical
This search will list all of the incidents that occurred containing the chemical name that you have chosen.
 
Incidents by Company
This search will list all of the incidents that have involved the company you have chosen.
 
Incidents by Incident Number
This search will list the specific incident that you have entered the IncNumber identification number for.
 
Incidents by Responsible Party
This search will list all of the incidents that have involved the responsible party you have chosen.
 
Incidents by Street
This search will list all of the incidents that occurred on the street that you have chosen.
 
Incidents by Zip Code
This search will list all of the incidents that occurred within the zip code that you have chosen.
 
Occurred at a Fixed Facility
This search will show all of the incidents that have occurred at a fixed facility.
 
OccurredDuring Transport
This search will show all of the incidents where the source of incident involved a mode of transportation: commercial vehicle, car, train, ship, or aircraft.
 
Reported DrugLabs
This search will show all of the incidents that were reported as drug labs. This listing is not the official list of sites that have been determined to be Unfit for Use. Contacting the Department of Human Services’ Clandestine Drug Lab Cleanup Program at 503-731-4012 can have information on the official listing.
 
If you find that you need more information on an incident, requests for incident reports must be in writing. They can be faxed to 503-373-1825, mailed to 4760 Portland Rd. NE, Salem, OR 97305-1760, or e-mailed to SFM.CR2K@state.or.us. A copy of the original report will be sent to you by US Mail or via shuttle for state agencies. Please allow a two-week turn around time on all requests. In requesting a report, please list the incident number and county.
