2008-2010 OT and OTA Renewal Forms
The renewal forms will first be sent out in early March, 2008 by e-mail notice with an explanation of how you can get online and renew your license.
 
For those who do not wish to renew or do not indicate that you will renew online, paper copies will be sent out March 17th to the address in your data base.   Renewal forms must be postmarked by May 1, 2008 to avoid the $50 late fee.
 
A random sample of licensees will be required to have CE AUDIT of their classes.  This will be clearly indicated on the renewal form.  Just send in the CE audit form and copies of certificates so they can be approved.

The OT Licensing Board now has two-year licenses, which are renewed in even years.  Current new license are  valid to May 31, 2008!   The next licensing period will be May, 2008 through May, 2010.
 
You can click on the application forms above or below; just scroll up or down and then click on it to open the application form.  BE sure to click on the OT or OT Assistant one you need.  
 
We do not prorate fees.  However, depending on whether you get your license before renewals start in March of even years, you can get a few months free and the license expires in May, 2010.  If you wait until March, 2009, then you can get a one year license valid until May 2010. 
 
In July 2007 the fees were changed to: 
 
The fee for an initial Oregon occupational therapy license is $100
The two-year renewal fee for the occupational therapy license is $170
       ($85 per year)  (reduced from $100 a year )
 
CLARIFICATION NOTE: An application form for either OT or OT Assisant license is for the first year of  licensure.  If you are applying at the beginning of a two year licensure period you will pay the initital fee and the second year fee; if you are applying during the second year of the licensure period, you will pay only the application fee and not the second year fee.  Please see the application form.
 
The fee for an initial Oregon occupational therapy  assistant license is $70
The two-year  renewal fee for the occupational therapy assistant is   $120  ($60 per year) (reduced from $70 a year )

Our goal for issuing licenses after receipt of all required documentation is to have them printed within three working days.  The part of the process that takes the longest is receiving verifications.  You can expedite the process for verification from NBCOT - see the information with the application form.  For the verification from whichever states you were licensed in the last five years, be sure to check whether they charge a fee.  Some applicants are waiting for a verification and then find out there is a fee which was not paid and no verification was mailed to Oregon. 
 
If you have used a different name and some of the documents might come in another name please make that clear on the front of your application form!  Otherwise that document will be filed in a separate file.  If you have had a name change, include a copy of the legal document showing the name change. 
 
If you have practiced or been licensed for up to three years, be sure to show clearly that you have updated your Continuing Education points (15 points for each year).

If you have not been licensed for three years or more then you must complete a Re-Entry program.  See the new "Re-entry Program"  as a means of re-education and updating your skills.  For additional information, contact the director by calling the office at 971-673-0198 or by e-mail at Felicia.M.Holgate@state.or.us  

Limited Permits:
Limited Permits for OT and OT Assistants are available
A Limited Permit is available for recent students of OT and OT Assistants who have registered to take the exam and are awaiting to take the exam and get their results.  The costs is $25 and can only be issued once. 
 
Limited Permits applicants must show evidence of being approved to take the certification exam (NBCOT).  The limited permit is valid for 60 days from the issue date of the eligibility letter you receive from NBCOT to take the exam.  It is NOT until the end of the eligibility date, but the issue date of the eligibility letter.  In other words, you must be sure to plan to take the exam early enough to get the exam results before the expiration of the limited permit license.  Otherwise your LP will expire and you will not yet have the results in time to be issued a regular license and you will not be able to work. 
 
Limited Permits are not issued to applicants who have taken and failed the exam.  A person who fails the exam must immediately surrender the permit upon receipt of exam scores.  Limited Permits are issued only once.
