| License Verification |

Do you need a License Verification?
Click here to verify a license online . The information for verifications online does constitute a "primary issuing source" for purposes of license verification. This verification will show whether the licensee has been disciplined by the Board.
When you open up the verification pack you can click on the Name and open to also look for the licensee under last name, first name, license number, or city. You can also look on the second box for specialty.
You can also request a verification of licensure, by telephone at 971-673-0198 or by e-mail: Felicia.M.Holgate@state.or.us or by writing to the Board office. You can also send a fax to 971-673-0226.
There is no charge in Oregon for license verifications.
The Board verifies the license number, whether it is current, the date of initial licensure, and whether there have been any disciplinary actions taken. Requests for written license verifications can also be requested by phone, e-mail or letter and can generally be sent out within a few days.
Questions regarding whether the licensee "is in good standing" are less public. Oregon law requires all information obtained during a complaint be kept confidential, including the fact that there is an on-going investigation (except in rare instance where there is a showing by clear and convincing evidence that disclosure is in the public’s interest). However, once the investigation is complete and the Board, by a majority vote, has taken disciplinary action, a copy of the action is available on request. These requests generally come from other state licensing boards, insurance carriers, and potential employers.

As of August 2007 there are just over 1500 licensed Occupational Therapists in Oregon. Included in that total are 251 OT Assistants.

Licensee lists are public information and are available upon request. Persons may order a list of licensees by sending in a written request, stating whether list is to be in zip code order or alpha order, along with appropriate payment.

If it is a name change be sure to include a copy of the legal document that changed your name and exactly how you wish it to appear (both names, middle initial etc.?). The document can be a copy of the divorce paper, marriage certificate or name change document.
