To submit any changes to your name, address, telephone, e-mail address:
Please get changes to your contact information to us in writing.
It is best to send an e-mail to Felicia.M.Holgate@state.or.us
You can also fax it to 971-673-0226, or mail it to: OT Licensing Board,
800 NE Oregon St. Suite 407, Portland, Oregon 97232.
If it is a name change be sure to include:
- A copy of the legal document that changed your name;
The document can be a copy of divorce papers, marriage certificate or name change document.
- Exactly how you wish the name to appear
Do you want both names, a middle initial ?
- Other changes to address, telephone or e-mail.
Please take care to do this. We sometimes get a name change, then are told the licensee actually wants a former name instead of their middle initial so want a new copy of the license and after it is mailed, it is returned because the address has changed.
This can be very frustrating and take a lot of our time. At this point we do not charge for printing a new license. If it starts taking a lot of time we will need to start charging a fee.

| LICENSE VERIFICATION |

Do you need an OT license verified?
There is NO charge in Oregon for license verifications.

The license verifications does constitute a "primary issuing source" for purposes of license verification. You can click on the Last name, First name, License Number, or City, or scroll down to the second box for specialty areas.
If you need a written verification form sent to another state, it is best to request the verification of licensure by e-mail. Send the request to Felicia.M.Holgate@state.or.us giving the full address where the verification needs to to. If you cannot send the request by e-mail then you can call 971-673-0198, or fax 971-673-0226 or by writing to the Board office.
We try to send verifications out within a few days. The Board verifies the license number, whether it is current, the date of initial licensure, and whether the board has disciplined the licensee. We will try to notify you by e-mail when the verification is sent.
Note: Oregon law requires all information obtained during a complaint be kept confidential, including the fact that there is an on-going investigation (except in rare instance where there is a showing by clear and convincing evidence that disclosure is in the public’s interest).
However, once the investigation is complete and the Board, by a majority vote, has taken disciplinary action, a copy of the action is available on request. These requests generally come from other state licensing boards, insurance carriers, and potential employers. See more information on the Complaints/Investigation tab.

Do you need an address list of Licensees?
Licensee lists are public information and are available upon request for a fee of $25. Persons may order a list of licensees by sending in a written request, stating whether list is to be in zip code order or alpha order, along with the $25 payment.

| SUPERVISION STATEMENT |

OT Assistants working in Oregon must have a signed Statement of supervision
Look under Supervision or for a Statement of Supervision form click here.
A new rule deletes the requirement that the Statement of Supervision be notarized.
During the 2010 renewal the Board hopes to have you fill out the supervision form online as part of the online renewal application.

As of October, 2009 there are 1674 licensed Occupational Therapists in Oregon. Included in that total are 268 OT Assistants.
