This is the easiest way to check the license verification of an Occupational Therapist or Occupation Therapy Assistant. This information does constitute a "primary issuing source" for purposes of license verification.

There is no charge for licensure verification in Oregon.

As of August, 2007 there are just over 1500 Occupational Therapists in Oregon. Of this total 251 are OT Assistants.

It is important that the OT Licensing Board has current information.
For address, telephone submit the changes to us directly online by e-mailing Felicia.M.Holgate@state.or.us
If you name changes the Oregon Occupational Therapy Licensing Board can issue you a new license, but you need to mail or fax (971-673-0226) a copy of the legal document showing the name change (marriage certificate, divorce) to:

Please be sure to indicate exactly how you want the name change to show (with middle initial or both names etc.). After you receive the new license you will need to mail in your old one so that we can void it in our records. Be sure to send in all the changes: telephone, address, work etc.

Remember: It is your responsibility to keep us advised of your current address and telephone and place of work. This is especially important when renewal notice are mailed out to the last known address given to us at the office.
