Eligibility to Retire
 
 
The OPSRP Pension Program Pre-Retirement Guide provides instructions and help to complete your Service Retirement Application. Additionally, it will guide you step by step through the retirement process.
 
You should also read about retiring from the IAP. IAP Retirement forms can be found there.

 Required reading. *Sign and submit Acknowledgement form unless you waive your right to 30-day review period on your retirement application.
OPSRP Lump-Sum Distribution Withholding Election  Optional
 Required for lump-sum distributions.
W-4P 2009
 Required
 Federal tax witholding form
W-8BEN Required Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding
W-9 Required Request for Taxpayer Identification Number and Certification
Estimate Request Optional Request estimate.
Additional Estimate Request Fee Agreement  Optional Fee agreement for requesting more than two estimates a year
Verification of Age Documentation
Required		 List of acceptable documents for verification of age, which is required before payment of benefits.
Rollover Eligible Distribution  Optional Form to roll over benefit.
Direct Deposit Form
 Optional
 Set up or change automatic deposits of your benefit.
Special Power of Attorney  Optional Power of attorney form to appoint others to request, change information, or make decisions.
 
911 operators can retire after 25 years of service. To ensure you meet this requirement, complete this form, and submit it to PERS.
