W4-P 2009 Required Form to withhold income tax from retirement benefit.
W-8BEN Required Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding
W-9 Required Request for Taxpayer Identification Number and Certification
Federal Tax Information and Acknowledgement Form  Required  
PERS Lump-Sum Distribution Withholding Election  Optional Form to elect lump-sum withdrawal withholdings.
Variable Election: Retiring member  Required* *Required only if you have a variable account balance at retirement.
PERS: Rollover Eligible Distribution Required* *Required only if you elect a lump-sum payment option and you wish to rollover the benefit.
Verification of Age Documentation
Required
List of acceptable documents for verification of age, which is required before payment of benefits.
Direct Deposit Form Optional Set up or change automatic deposits of your benefit.
Direct Transfer Rollover Acceptance Optional Roll over benefit to qualified employer or compensation plan.
Estimate Request Optional Request estimate. (Click here for important information if you are a school or academic employee.)
Additional Estimate Request Fee Agreement  Optional Fee agreement for requesting more than two estimates a year
Special Power of Attorney Optional Power of attorney form to appoint others to request, change information, or make decisions.
Beneficiary Non-Spouse Rollover-Eligible Distribution Optional  
 
911 operators can retire after 25 years of service. To ensure you meet this requirement, complete this form, and submit it to PERS.
 
If you are a police officer, firefighter, educator, or judge member, you may need to complete the forms listed below.
