PERS provides all online forms and publications in .pdf format. To view them, you must have the most recent version of Adobe Reader®. Click here to download Adobe Reader®.
You can fill out these forms on your computer. Use the TAB key to move from one field to the next. You can also use your mouse to move from field to field. To do this, position the cursor in the field, and click with your mouse.
Print the form before you fill it in online to ensure your printer has the capability of printing the form correctly.
Each form has its own instructions. Make sure you read and follow the instructions completely. Sign and date the form where indicated. Incorrectly completed or unsigned forms will cause a delay in processing your information.
All forms may be mailed to OSGP; some forms may be faxed.
