Form I. Annual Report for telecommunications utilities or cooperatives with certificates of authority under ORS 759.025; may also have certificates of authority under ORS 759.020 and ORS 759.050.
Form L. Annual Report for Competitive Local Exchange Carriers (CLEC) with certificates of authority under ORS 759.020 and ORS 759.050 to provide intraexchange service only or to provide intraexchange and interexchange service.
Form O. Annual Report for telecommunications utilities or cooperatives with certificates of authority under ORS 759.025; may also have certificates of authority under ORS 759.020 and ORS 759.050.
