Fill out the form and mail to the Agency with the appropriate fees. A late fee of $30 will be assessed.
Active renewal fee will be $260.
Inactive renewal fee will be $140.
Late renewal applications must be postmarked within one year after the expiration date. For example, if your license expired January 31, 2011, the renewal application must be postmarked or turned in to the Agency no later than January 31, 2012.
