How to Apply

 

APPLICATION

Each applicant seeking to become licensed must meet and/or complete the requirements for the Certificate of Clinical Social Work Associates (CSWA) (ORS 675.530(4); (OAR 877-020-0009). The applicant for Associate must complete and submit the application.  Please take note of the sections within the application that may or may not be applicable.  Please see the "Previous Experience" section of this page to see instructions for prospective CSWAs who have accrued supervised hours in another jurisdiction.

CSWA PLAN OF SUPERVISION (PG 8 - 10)

OAR 877-020-0012 REQUIRENTS OF SUPERVISION
(b) The supervisor of the associate:
(A) Must have completed two years of licensed practice.
(B) Must have completed and reported to the board at least six hours of continuing education hours described in OAR 877-025-0006(1) or (2) in techniques of supervision within five years prior to commencing the supervision of an associate.
 

*Please assure that proof of completion of the 6 credit requirement is available to The Board - if we do not have this on file, you will need to submit the certificate. 

 

*Please assure that all required signatures and initials are in place (this includes the administrative supervisor; the person you report to directly in the workplace).

*Keep in mind that you must meet with your supervisor bimonthly, for one hour each meeting; however you may meet more often if you wish.

 *It is best to set up your Plan for the full two years, but it is acceptable to begin your Plan with just individual or group supervision – remembering that of the 100 hours of required supervision, 50 must be done individually. Upon Board approval of your Plan, you will receive official notification.

*To change your plan, refer to the Forms section of our website.

*If an applicant is not located within a 50-mile radius of an LCSW, it is possible that the Board can approve supervision by a non-LCSW mental health professional.  Please submit a written request detailing your situation if this is the case, and submit a CSWA SUPERVISION EXCEPTION REQUEST

 

Refer to OAR 877-020-0010 (located in DIVISION 20 of the Oregon Administrative Rules) for rules regarding a Plan of Supervision.

 

OREGON STATUTES ADMINISTRATIVE RULES EXAM

Together with Oregon's Rules and Statutes for reference, this is an open-book, self-administered quiz on the Rules and Statutes governing Oregon social workers. 

Once completed, the exam should be mailed, faxed, or emailed back to the Board's office for scoring. As applicable, the Oregon Jurisprudence Exam may be required a second time if there are any changes to Oregon's Rules and Statutes prior to licensure.

It is the policy of the Board that all applicants pass the Jurisprudence exam prior to issuance of a license or certificate.

FINGERPRINT ID FORM & INSTRUCTIONS
 
The Board requires all applicants to participate in a nationwide fingerprint-based criminal records check.  Applicants have the option of either having fingerprints inked and returned to our office with the ID form, or completing a live scan transmission.  Live scan transmission forms should be taken to the live scan agency to ensure that background check results are routed correctly.  Please read the instructions with the fingerprints documents carefully.
 

OFFICIAL TRANSCRIPT

An official transcript is required as part of the application process and is to be mailed directly from your graduate school to the Board office at 3218 Pringle Road SE, Suite 240, Salem, OR 97302.

Please note on your initial application if the name printed on your transcript differs from your current, legal name.

Previous Experience

If an applicant has accrued Out of State supervised clinical social work experience that meets the requirements of OAR 877-020-0010, the Board may consider this Clinical work experience and supervision hours as part of your Plan.

The PREVIOUS HOURS COMPLETED IN ANOTHER JURISDICTION and VERIFICATION OF LICENSE IN ANOTHER STATE sections of the application should be filled out by the applicant and supervisor(s) to document hours accrued outside of the Oregon Board's jurisdiction. 

 

Oregon Hour Requirements:

Total Worked Hours: 3500

Direct Client Hours: 2000

Supervision Hours: 100*

*50 of these must be Individual

 

Acceptance of any previous practice and supervision is at the discretion of the Board.

Exam Information

CSWA candidates are eligible to take the ASWB National Clinical Exam upon completion of 75 supervision hours.  
 

Approval from the Board is required in order to sit for the exam.

Please submit a brief request in a written format (Letter, fax, email) to our Board Office.

 
The National Clinical Exam is administered electronically by Pearson at test sites in Portland, Beaverton, Salem, LaGrande, and Medford. Test centers are open weekly. Specific information about the testing is provided at the time of registration with the ASWB. A passing score of 70 is required. Instructions on the ASWB Website will provide all necessary information on how to make a reservation. 
 
Upon successful completion of the National Clinical Exam, notification is sent to the Board office by Pearson. An official license will be issued after payment of the appropriate license fee and completion of remaining hour requirements.

ASWB may assist those exam candidates for whom English is a Second Language by providing appropriate accommodations at the testing site. Board approval is required prior to being granted ESL accommodation. Contact the Board office for further information.
