How to submit a complaint regarding
Portland's sick leave ordinance
BOLI's Wage and Hour Division accepts complaints that are filed within 180 days of the date on which the alleged violation first occured - if you feel you have been denied your rights under Portland’s sick time requirements please fill out and submit a complaint form (click on the links listed below). You can fill it out online and send it to us electronically or you can print out a hard copy and mail it to us at:

Please note: The complaint forms above have to do with Portland’s sick time ordinance ONLY. If you have a claim for unpaid wages you must also complete a wage claim form in addition to this complaint form. The Wage Claim form is available at any BOLI office or online at: www.oregon.gov/boli/whd/docs/wageclaim.pdf.
