| General Process / Timeline |
|
- The general timeline for processing the application is usually 5-7 working days from receiving all required items and depositing the payment. Verifications of licenses you have held in other states frequently take several weeks to obtain, so please request these well in advance of the time you need to be licensed. Also, if you have disciplinary or criminal history, the Board will need to review these documents and this may delay approval of your completed application.
- It is important that you not practice speech-language pathology or audiology, or represent yourself as an SLP, audiologist, or SLPA until your license is issued. If you hold a conditional license, you may not practice independently until you are issued your regular SLP license. You can check the "Licensee Directory" (on the tab to the left) to see if your license has been processed (the online directory is updated each hour), or call or email the office regarding your application status if more a week has passed since you submitted your application.
- The pieces of the application file do not need to arrive all at once. We will set up a file for you upon receiving one of the above items and collate the others as they arrive.
- Applications that remain incomplete six months after their receipt are considered to have been abandoned, and are destroyed. If this happens, you will have to complete a new application, re-send all supporting materials and pay the application fee again if you wish to obtain a license. The application fee is non-refundable.

| Criminal and Professional Discipline History |
|
If you answer "yes" to any of the criminal/professional discipline history questions, you must submit information about these incidents for Board review.
- A copy of the related court proceedings, police reports and/or Board order for each charge, conviction and disciplinary action;
- A copy of the document(s) evidencing that you met your legal obligations as ordered; and.
- A personal statement (written narrative) describing the surrounding facts and circumstances.
Applications with criminal or disciplinary history take longer to process as the Board needs to review the information to make a judgment on fitness to practice. It is important to answer the questions regarding criminal or disciplinary history completely and truthfully. Failure to do so represents unprofessional conduct as defined in Oregon Administrative Rules 335, Division 5, "Professional and Ethical Standards" and may result in denial of your application or imposition of professional discipline if it is discovered after your license is issued. |

| License Period |
|
- Regular SLP, Audiologist and SLPA licenses are valid for up to two years. All regular licenses expire on January 30th of the next even-numbered year after issuance. The renewal deadline is one month earlier--December 31st of odd-numbered years. All regular licenses are currently expiring on January 30, 2014, and will need to be renewed by December 31, 2013.
- Conditional SLP licenses are valid for one year from the date they are issued. However, a conditional license may only be used during the time that an SLP is practicing under the supervision of another SLP who meets Board qualifications as a supervisor. You may not practice independently until you are issued your regular SLP license. A conditional license may be renewed once if more time is needed to complete requirements for a regular license. | Foreign Education/English Language Proficiency |
|
If your qualifying education was received at a foreign institution, you will need to submit the following:
- Degree Verification from a Board-approved credential evaluation agency
- SLP applicants who do not have (or haven't ever had) ASHA CCC's will need to provide evidence of at least 1,260 hours of supervised post-graduate clinical experience, as well as passing score on the PRAXIS exam.
If English is not your first language, then you will be required to submit evidence of a passing score on the TOEFL and/or TSE to demonstrate English language fluency. Specific required scores vary based on whether they were administered via the internet, paper, or computer-based versions. |

Official Transcripts for Master’s Degree You may send these in if they are in a sealed envelope from the university; otherwise they need to come directly from the university to us. If your degree is from an ASHA-accredited program, we only need the Master’s degree transcripts. If it isn't, we will need you to supply documentation of your clinical practicum hours, and may require additional information about coursework and other accreditations. |

If you have or ever held ASHA CCC’s (Certificate of Clinical Competence): You need to request verification from ASHA. We will accept verifications faxed from ASHA. If you have never held ASHA CCC’s: You’ll need to send:
- Copy of your NTE Praxis scores for SLP (score must be 600 or over)
- Was your school accredited by ASHA when you graduated?
- Documentation of completion of appropriate post-graduate supervised clinical experience, in accordance with ASHA requirements (currently 1,260 hours).

Professional Development (PD) There are three choices for professional development (see page 4 of the license application):
- If you have completed your graduate program within the 12 months preceding the date you apply: No professional development hours are needed; check this box and sign and date the bottom of the page. You do not need to fill in the grid to report PD hours. [Note: if you renewed your conditional license, you will need to meet PD requirements in #2 or #3 below.]
- If you have 30 hours completed within the 24 months preceding the date you apply: Complete the reporting form, and be sure to include copies of your certificates of attendance for all activities you report. Read our professional development page for more information on acceptable professional development.
- If neither of the options above applies, you can apply without reporting any prior professional development, but must agree to submit evidence of completion of a minimum of 10 hours within 12 months following the date of initial licensure. Be sure to sign and date the bottom of the page.

Verification of Good Standing From Other Licensing Bodies We need to have a letter of good standing from every other state or other jurisdiction (including Oregon's Teacher Standards and Practices Commission and other states' educational licensing bodies) in which you have held a professional license, regardless of how long ago it was. This verification must come from the jurisdiction to us; the Board does not accept information in on-line directories. If the state does not hold a record of your licensure anymore, we will accept a letter from them stating as much.

Official Transcripts for Credit Requirements We need official transcripts showing 45 quarter or 30 semester hours of general education credit, and official transcripts showing 45 quarter or 30 semester hours of technical credit. It is not necessary to have a degree conferred, just the earned credit. Please note that having a SLPA certificate from Chemeketa Community College or another school may not automatically grant the general and technical credit that you need. Be sure to evaluate your credits to make sure that you have 45 technical and 45 general credit hours total. Link to what courses are acceptable

Documentation of Satisfactory Completion of Supervised Practicum Experience You need to submit one or more logs to document a total of at least 100 hours of supervised clinical interaction (not just passive observation time). You may have more than one supervisor for this experience, but each supervisor must be licensed by this Board or hold their ASHA CCCs. You may use Supplement 4 of the SLPA Certificate application, or use your own form, as long as it includes the following required elements:
- Date
- Description of clinical activity
- Amount of time spent
- Supervisor’s initials for each date
- Supervisor's signature and license or ASHA certification number
You also must submit a Clinical Competency Checklist completed by each practicum supervisor. See Supplement 3 of the application form. |

Official Transcripts for AuD or Master’s Degree You may send these in if they are in a sealed envelope from the university; otherwise they need to come from the university to us. If you earned an AuD from an ASHA-accredited school, then we will need an official transcript and a copy of your Audiology Praxis test report showing a passing score.
If you completed your Audiology training prior to August 1, 2007, your transcript must document that you earned a Master’s degree from an ASHA-accredited program with at least 75 semester credits in Audiology. In addition, you will need to submit documentation of your successful completion of post-graduate supervised clinical experience (currently 1,820 hours), and a passing score on your Praxis exam.
