Online Application (Adobe PDF Reader ONLY)
The application Instruction Booklet and the application forms are seperated by entity. Please read the Instruction Booklet as it contains valuable
information to help you complete the application correctly (such
as choosing your entity, bonds, insurance, fees, etc.).
To submit the application online or print (Use Adobe PDF Reader ONLY)
Although there are multiple PDF readers available, this form is designed to work with Adobe software only (to download for free, click the adobe reader icon on the bottom of this page). Each
application can be submitted by clicking on "Submit Your Application"
button at the bottom of the last page. Upon submitting, you will receive an automated email letting you know your application has been received.
Here's how to submit your application online using Adobe PDF reader or acrobat:
1. After reading the Instruction Booklet. Click on the link to open your selected application.
2. Save the application to your computer.
3. Open the saved application.
4. Complete the application and hit submit or print at the bottom of the application. You will receive an automated response letting you know your application has been received successfully.
5. The Application Payment and Signature form can be printed then mailed, faxed (secured line) to 503.373.2155 or dropped off to the CCB.
Attaching the completed application to an email manually:
1. After reading the Instruction Booklet. Save the document to your computer.
2. Complete the document and save your completed application.
3. Create an email to ccb.app@ccb.state.or.us
4. Attach your completed application and hit send. You will receive an automated response letting you know your application has been received successfully.
5. The Application Payment and Signature form can be printed then mailed, faxed (secured line) to 503.373.2155 or dropped off to the CCB.

Required Notices for Residential Construction (Chart)
This chart summarizes the three notices a contractor must give to the homeowner when
contracting for residential construction work. (Consumer Protection Notice, Information
Notice to Owner about Construction Liens and Notice of Procedure) Copies of each of the forms are available to download below.
Consumer Protection Notice
This notice is required to be given to the property owner by the contractor at the time ofcontract.
Information Notice to Owner About Construction Liens
This notice is required to be given to the property owner by the contractor at the time of
contract.
Notice of Procedure
This notice is required to be given to the property owner by the contractor at the time of a written or verbal contract. It replaces the form "Owner's Duty to Contractor in the Event of a Construction Dispute"
Homebuyer Protection Act (HPA) Notice of Compliance Form
This document explains how the HPA, adopted by the Oregon Legislature in 2003, protects buyers against construction liens filed in county records after the sale of the property where such liens arise out of new construction, additions or remodeling within 90 days of the date of the sale.
Notice of Right to a Lien
This notice must be delivered to property owners by subcontractors, material andequipment suppliers before the construction is complete, though failure to deliver within 7
days of the project's start may reduce your lien claim.
EPA "The Lead-Safe Certified Guide to Renovate Right" | EPA "Remodelar Correctamente" (spanish)
Contractors are required to provide this pamphlet prior to beginning renovation activites on"target housing" described in Oreon's lead-based paint requirements.
Model Features for Accessible Homes
Contractors constructing new homes may provide this form containing features for accessibility to the home owner. It is not a required form.

This addendum is provided by the CCB to assist contractors in complying with certain contract term mandates. Contractors may opt to utilize or include their own language into the contract that complies with the law.
Maintenance Schedule & Moisture Intrusion Information
Contractors must provide information on moisture intrusion and water damage, and a maintenance schedule to the homebuyer when the contractor constructs a new residential sructure.This document may be used to comply with the requirement. Contractors may develop and use their own as long as it contains the information required under OAR 812-001-0240.
