The Oregon Board of Dentistry has mailed out our random audit letters. CE Audits for those selected for random audit were mailed in early December, a month prior to the renewal notices. Only 15% of the dental licensees due to renew are audited by the Board. This is a random audit and the individuals selected are chosen by a computer program. It IS statistically possible to be audited multiple years in a row.
Dental Renewal reminder postcards were mailed January 23, 2013. You do not need a postcard to renew your license if it is due. Just follow the link below on this page from January 23 on. You will need to provide the following information in order to access your renewal via the Boards' online renewal process:
•Last Name as registered with the Oregon Board of Dentistry
•Last 4 digits of Social Security No.
•Oregon Dental License No.
Once you have accessed your online renewal application you will be able to complete the process entirely online. If you should need to stop in the middle of the process your information up to the last completed page will be saved and you can return at a later date to complete the process.
PAYMENT OPTIONS
•Pay online with a US issued Visa or MasterCard at time of renewal
•Print completed renewal and mail it to the PO Box with a check or money order.
◦Printed forms MUST be signed and dated or you will not be renewed.
◦Payment MUST accompany printed renewal form. Renewals received without payment will be returned to licensee for payment. Your license will NOT be renewed until payment is received with the renewal application.

Dental Licenses for the current renewal cycle will expire March 31, 2013. If you do not renew your license you cannot practice! Below is a link that allows you to check your current license status which also shows your license expiration date. Please keep in mind that if you just renewed recently there may be a delay in the posting of current information.
ACTIVE DUTY MILITARY
Oregon Dental licensure fees are waived for those licensees who are active duty military. For those individual seeking waiver of fees, you must select the 'Active Duty Military' option after completing the online renewal when asked how you would like to pay. In lieu of payment you must send in documentation of your active duty military status.
VOLUNTEER LICENSES
Oregon Dental licensure fees are waived for those licensees who hold a volunteer dental license as long as the qualifications are met. You MUST have verification of 80 total volunteer hours (40 hours per calendar year). In lieu of payment you must send in the completed printed electronic renewal forms with signature in the 3 locations located on the renewal form:
•2013-2015 Dental Renewal Form
•Dental/Dental Hygiene Limitation of Liability (ORS 676-340)
•Verification Certifying Completion of 80 hours of volunteer work (40 per calendar year)
READY TO RENEW NOW?
- Licensees are personally responsible for their license renewal. That means no one other then the licensee should be answering the renewal questions.
- There will be no exception for the online process. If you do not have access to a computer, you can visit a local library, a family member or friend, or if in the Portland-Metro area, you can visit the Board office.
