Application and eligibility determination services
Each Community Developmental Disabilities Program (CDDP)
is required to provide assistance to residents of their service area in applying for developmental disability services. The CDDP of residency of an individual applying for services must make the determination of the individual’s eligibility to receive developmental disability services.

Case management or service coordination services
Case Management or Service Coordination services
are identified as eligibility determination, evaluation and coordination of developmental disability services, delivered to individuals in need of such services, as funded by the Department. The purpose of Case Management Services is to provide a fixed point of entry to the developmental disability services arena. Each CDDP is required to provide case management or service coordination services to individuals who are eligible for and desire to have the services. The CDDP may choose to provide services to individuals awaiting eligibility determination, who reside in the CDDP service area at the time they apply for services.
Service Planning Support
must assist individuals in the planning of services
. Service plans for individuals must be developed in a manner that addresses issues of independence, integration and productivity. Plans must be designed to enhance the quality of life of the person with developmental disabilities, and must be consistent with the principles of personal control and family participation, choice and preferences, identification and reduction of barriers to services, protection from harm, and for children, maintaining family connections.
Abuse investigation and protective services
must provide or arrange for protective services in cooperation with other agencies that have authority to investigate allegations of abuse or neglect for adults or children. Each CDDP is required to assure that abuse investigations for adults are appropriately reported and conducted according to state statute and administrative rules.
When there is reason to believe a crime has been committed, a report must be filed with local law enforcement. Any suspected or observed abuse of children should be reported directly to Child Welfare Services, or to local law enforcement.

Foster Homes licensing and certification services
Each Community Developmental Disabilities Program (CDDP) is responsible for the recruitment of Foster Home applicants, and assisting applicants with the licensing or certification process. For more information on Foster Home licensing or certification, contact your local CDDP.

Information and Referral Services
Each CDDP is responsible for supplying information on developmental disabilities and providing referral services to individuals, their families and interested others.

Comprehensive In-Home Support Services for Adults
Comprehensive In-Home Support Services
enable individuals with developmental disabilities aged 18 and older to continue to live in their own homes or in their family homes. Each CDDP is required to ensure the availability of Comprehensive In-Home Supports for those individuals for whom the Department has funded such services.

Child and Family Support Services
Child and Family Support Services
enable eligible children with developmental disabilities, 17 years of age and younger, to continue to live in their family homes. These services supplement those things typically provided to a child by the family and other public or private sources. Each CDDP is required to ensure the availability of a program for Child and Family Support Services. The CDDP Case Manager will coordinate with the family or the legal guardian in the development of the child and family service plan.

include a variety of direct services, supports and equipment, designed to address the specific issues leading to or contributing to a crisis situation, and can only be provided when no alternative resources are available. The purpose of Crisis Services is to maintain or return adults to community placements, or to maintain a child in his/her family home, or temporarily place the child out of the family home. The CDDP must provide crisis services only to those individuals who meet the specific eligibility requirements to receive crisis services.
Crisis services are delivered to adult individuals who are eligible for, and at imminent risk of civil commitment under Oregon Revised Statute (ORS) Chapter 427
, or who are receiving temporary services in response to having been committed under ORS 427. These services are also delivered to children with developmental disabilities who are at imminent risk of out-of-home placement.

Nursing Facility Specialized Services
Nursing facility specialized services
are individualized habilitation services for persons with developmental disabilities residing in nursing facilities. These services are to be integrated with, but are in addition to the care the nursing facility is to provide. The CDDP Case Manager must coordinate these services with the individual, the staff of the nursing facility and other service providers to arrange for the services.

Centralized waiting list
Each CDDP must maintain a current waiting list for eligible adult individuals living within the geographical area of the CDDP and appropriate for admission to Comprehensive Services. Individuals are to be added to the waiting list following written determination of their specific service and support needs. The CDDP must assure that individuals are admitted to programs from the waiting list, using a fair and equitable process, per DHS policy.

Complaints, Grievances and Appeals
The CDDP is expected to assist individuals and family members in filing any complaints, grievances or appeal requests. Each CDDP is responsible for maintaining a log of all complaints and grievances received, and must have policies and procedures in place for processing such matters. There are specific timelines and processing requirements in place for responding to and resolving complaints, grievances and appeals.
