Preadoption birth record
As a result of Measure 58, an initiative passed by Oregon voters in 1998, an Oregon born adoptee age twenty-one and older can receive their Preadoption Birth Record, or original birth certificate. Please call the Oregon Center for Health Statistics (and Vital Records) at 971-673-1190 to request the order form or you can download the forms from their web site.

 

Can birth parents state their preference about personal contact with the adoptee?
The answer is yes. The 1999 Oregon Legislature passed HB 3194 to provide a way for birth parents to file a form with the Oregon Center for Health Statistics (and Vital Records), stating their preference about personal contact with the adoptee. The completed form, Contact Preference Form for Birth Parents of Adopted Children (CPF), is filed with Vital Records, and is placed in the file. The CPF would be mailed to the adoptee if the birth record was requested. If the CPF is filed within six months of issuance of the Preadoption Record, Vital Records will forward the CPF to the address of the adoptee listed on the order form. The CPF allows a birth parent to indicate if they want contact, do not want contact, or contact through an intermediary. The CPF is available by accessing the Vital Records web site or by contacting Adoption Services Unit, Adoption Registry at 503-945-6643, and indicating your contact preference.

 

If the birth parent wants contact, or contact through an intermediary, he/she will complete the CPF, and return the form to:

If the birth parent does not want contact, he/she, in addition to the CPF, must fill out the form Birth Parent Updated Medical History (PDF) and file the form with the adoption registry, or with the private adoption agency involved in the adoption. Once the medical history form has been received by the appropriate registry, a notarized certification statement is issued, and both the notarized certification, and the CPF are forwarded to Vital Records.
