The Teen Advisory Board (TAB) is a statewide leadership opportunity for Teen Leaders involved in My Future-My Choice to develop leadership skills and act as the youth voice of the program. TAB members work with a local mentor to carry out a youth sexual health leadership project over the school year as well as give program feedback. Selected TAB members are required to attend two statewide meetings: the Leadership Kick-Off weekend in the fall and the Mid-Year Meeting in the winter. During these meetings, members will get the chance to meet with TAB members from around the state and share project ideas.
Applicant requirements:
- Have My Future-My Choice classroom experience as a Teen Leader
- Complete and submit an application by May 1, 2013
- Obtain and submit a completed Letter of Recommendation Form by a teacher, Classroom Facilitator, coach, or other adult by May 1, 2013

Mentor Information
- Have a desire to act as a local Teen Advisory Board Mentor
- Can meet monthly with a TAB member and attend required meetings
- Complete and submit an application by July 3, 2013
- Pass a background check
