CCO Resource Book

The key roles of Coordinated Care Organizations (CCOs) and the Long-Term Care (LTC) System:

CCO Role:
Health care delivery including preventive, early intervention and acute health services, behavioral health services, health services coordination and information sharing, care team coordination, use of non-traditional health workers (health system navigators, peer wellness counselors, community health workers), Person-Centered Primary Care Homes, after hours medical consultation.
 

LTC Role:
Area Agency on Aging/Aging and People with Disabilities (AAA/APD): Coordination and information sharing with CCO, LTC financial/service eligibility, LTSS authorization and placement (home and community based/nursing facility except when Medicare skilled), LTSS case management coordination and troubleshooting, Adult Protective Services, contracting for Medicaid LTC providers, Licensing and Quality Assurance, LTC Ombudsman. Eligibility and enrollment for Medicaid, Medicaid low-income co-pay.
