Last-minute Checklist
You can use this checklist or something similar you have created to help avoid last-minute crises.
Emergency information (whom and where to contact) and necessary forms - use the Family Care Book to organize.
Important information (routines for nap time, meal time, bath time, etc.) - communicate to the respite provider.
Medication (prescription and over-the-counter) - provide enough supply and check instructions on bottle.
Special items to help the person feel comfortable (diapers, adaptive equipment, eating utensils, special foods, toys, books, etc.) - Note in the Family Care Book.
Necessary clothes - show the respite provider where they are.
Personal items (toothbrush, deodorant, diapers, hearing aids, etc.) - readily available?
Pets - provide specific instructions regarding the pets) and the person receiving care.
Transportation - double check any necessary arrangements.
Respite provider's contact information (name, address, phone number, email address) - keep on hand.
Change of plans - notified the respite provider?
