​Becoming a Provider

 
Thank you for your interest in the Long Term Care Community Nursing Program. The intent of this program is to help caregivers and case managers develop the supports needed to help a person with chronic health care needs live in their own home or a foster home. Registered Nurses in this program provide:
  • A nursing evaluation, medication review and service plan for the person,
  • Health education for the person, care providers and/or family members,
  • Teaching for family members to perform tasks of nursing for the person,
  • Teaching and RN delegation of nursing care tasks provided by a non-family or paid care provider,
  • Technical assistance and visitation summaries to the case manager regarding the person's healthcare needs and
  • If authorized by the case manager care coordination with the persons medical and ancillary health providers.
Delegation is a critical aspect of this service. Experience providing nursing delegation or a pass score on the Department’s nursing delegation self-study test is required to meet the qualifications of this program. You can check your knowledge of this function by completing this test.
 
 
There are two ways to become a Medicaid provider of this service.
  1. In Home or Home Health Agencies- A Request for Application (RFA) process is posted through September 17, 2013 for qualifying In- Home or Home Health Agencies who want a personal/professional services contract with Oregon Department of Human Services Seniors and People with Disabilities Division (SPD) through June of 2015. Qualifications can be found in the Long Term Care Community Nursing OAR 411-048 (0210). Instructions on how to complete and submit an application are noted below.

  2. Recruitment- A Request for Applications (RFA) for qualifying ‘Self Employed’ RNs who have not worked in the program or who did not renew their contracts. Qualifications for self-employed nurses can be found in the Long Term Care Community OAR 411-048 (0210).
