Criminal Fine Account (CFA) Coupon for Municipal and Justice Courts
Municipal and justice courts use payment coupons to report to the Oregon Department of Revenue (DOR) the amount of funds collected for the CFA.

There are two categories of funds reported on this coupon.
This figure is monies designated as "replacement fine amount for unitary assessment" as described in ORS 153.633.
Other state fee:
This figure is comprised of all other monies payable to the state for deposit in the CFA.
Note that monies previously designated for Intoxicated Driver's Program Fund (IDPF) and Law Enforcement Medical Liability Account (LEMLA) will now be included in the "Other State Fee" category.
If no funds are collected for either or all categories
Submit a coupon for the month even if no funds were collected. If there were no collections for a particular program, enter -0- in the box for that program. If there were no collections for any program, enter -0- in all the boxes including the "Total" box, and mail the coupon without a payment.
Timely submitting -0- amount coupons helps us keep your account accurate, record future payment into the proper time period, and eliminates the mailing of a late payment notice.
Payment due date
Send your payment and coupon to Oregon Department of Revenue by the last day of the following month the funds are collected (for example, funds collected in January are due by the last day of February).
Payments become delinquent on the 10th of the month following the due date. If you send payments after this date, you may be charged interest.
