To get Publication 17½;

Include:

  • Name/business name.
  • Mailing address.
  • Phone number.
  • Number of publications requested.

Payment:

  • Publication 17½; costs $5 per copy.
  • Include a check or money order with your request.
  • You can also include complete credit card information.
