Electronic funds transfer

There are four different forms that deal with Oregon's Electronic Funds Transfer (EFT) program.
 
Combined payroll taxes
Two forms cover making EFT payments for combined payroll taxes, one for each method (ACH debit and ACH credit). Five tax types are included in this category. They include state withholding tax, state unemployment tax, workers' benefit fund assessment (also known as workers' comp.), TriMet transit district excise tax, and Lane County transit district excise tax. ACH Debit Authorization Agreement and Application for Combined Payroll Tax and Assessment, 150-206-029 This is a free service provided by the state. These EFT payments are initiated by using either a touch-tone phone or going through our secure internet site.
ACH Credit Agreement and Application for Combined Payroll Tax and Assessment, 150-206-030 This EFT payment method is coordinated through your financial institution, which pushes funds to the department. Your financial institution may charge various fees for this service.
 

Estimated corporation taxes
Two forms cover making EFT payments for estimated corporation taxes, one for each method (ACH debit and ACH credit). Two tax types are included in this category. They include state corporation excise tax and state corporation income tax. Go to Corporation Taxes for additional information about these taxes.
