The next Oregon Formulary and Jurisprudence examinations for licensure will be held August 9, 2013, for those who have passed or are eligible to take the NPLEX Part I, passed NPLEX Part II, and NPLEX Minor Surgery elective examination.

1:00pm - 2:30pm Formulary examination
3:00pm - 4:00pm Jurisprudence examination

Fees (must accompany application and fingerprint card):
$150 Exam application
$  50 Background check
Make check or money order payable to Oregon Board of Naturopathic Medicine.

You may download application materials below or contact this office to have an examination packet mailed to you.

The Board must have these items before you can sit for the exams:

1. Official Naturopathic School Transcripts
2. Diploma from Naturopathic school (can be a copy from your original) 
3. NPLEX transcripts showing passage of Part I,  AND NPLEX Part II, and NPLEX Minor Surgery examinations, if you have already taken these exams

Additionally the following are required for licensure

4. NABNE transcripts showing passage of all required examinations

5. Official Undergraduate Transcripts*
6. Fingerprint card/passing of background check*

* It's a good idea to request your undergraduate transcripts early and to submit your fingerprint card as soon as your application has been submitted; although both are only required for licensure.

** Please note that the Formulary and Jurisprudence State Exams are in addition to and separate from the NPLEX Basic Science (Part I), Clinical Science (Part II) examination and Minor Surgery add-on examination.

The License application is only available to those who have qualified for licensure by meeting the requirements set forth in ORS Chapter 685 and OAR Chapter 850.
