Supervisor Training 
 
Orientation to Residency Supervision- this is a self-paced training for psychologists who supervise residents in Oregon.  At the end there is a short quiz.  Three hours of CE credit will be awarded upon completion of this training.

Supervisor Registry

The registry is a list of Oregon licensed psychologists who are available to provide supervision services to residents, with information about their training and experience in the area of supervision.  To be a supervisor need to have been licensed and in active or semi-active status for at least 2 years. A supervisor must be competent in the clinical specialty areas in which the resident will be practicing and with the populations with whom the resident will be providing psychological services.

• Psychologist Resident Supervision Contract- this form should be completed by the resident and supervisor and submitted to the Board's office. It must be approved by the Board before beginning post-doctoral supervised work experience.
• Psychologist Associate Resident Supervision Contract- this form should be completed by the resident and supervisor and submitted to the Board's office.  It must be approved by the Board before beginning post-masters supervised work experience.
• Record of Supervised Hours (i.e. the Hours Log)- this form is used to keep track of supervised work hours as you go, and is submitted at the end of the residency. 
• Interim Resident Evaluation Form- to be completed by the supervisor every 6 months.

• Final Evaluation of Resident Form- to be completed by the primary and associate supervisor (together, if applicable) at the end of the residency, and submitted to the Board's office.
• Final Evaluation of Supervisor Form- to be completed by the resident at the end of the residency.  Please send in the attestation slip only.
• Submit your completed Hours Log(s).
