DHS|OHA Policies and Forms

Information Security and Privacy Training and Policy Requirements

DHS and OHA employees are required by law to protect the confidential information entrusted to us.

All new and existing DHS and OHA employees are required to take online privacy and security training.

DHS|OHA Information Security Policies

These are joint agency policies and apply to all DHS and OHA employees. See below for additional policy requirements.

DHS|OHA Information Security Policies

All employees must adhere to the DHS|OHA Information Security Policies. If you're a new employee you are required to review and adhere to the DHS|OHA Information Security Policies. If you're an existing employee you are required to adhere to all existing DHS|OHA Information Security Policies and review any policies that are new or have been updated since your last review.


OHA Privacy Policies

DHS|OHA-100-010 and DHS|OHA-100-11 are joint agency policies and apply to all DHS and OHA employees. See below for additional policy requirements.

OHA Privacy Policies

All OHA employees must adhere to the OHA Privacy Policies. If you're a new OHA employee you are required to review and adhere to the OHA Privacy Policies. New DHS employees in the following groups are also required to review and adhere the OHA Privacy Policies: Aging and People with Disabilities, Developmental Disabilities, Facilities, Internal Audits, Payment Accuracy and Recovery, Office of Program Integrity, Office of Business Intelligence, Office of Contracts and Procurement. If you're an existing OHA employee or existing DHS employee in one of the groups listed above you are required to adhere to all existing OHA Privacy Policies and review any policies that are new or have been updated since your last review.

All DHS employees must adhere to the DHS Privacy Policies. If you're a new employee you are required to review and adhere to the DHS Privacy Policies. If you're an existing DHS employee you are required to adhere to all existing DHS Privacy Policies and review any policies that are new or have been updated since your last review.


All new DHS and OHA employees must certify they have read and agree to these policies and procedures by submitting to HR a signed and approved DHS|OHA Policy and Procedure Summary (MSC 2400).

Privacy Forms

These forms inform clients of how their private information will be protected and provides them a variety of ways to specify their privacy requests. Many forms are available in alternate languages. Search the DHS/OHA forms database for alternate language versions.
