Training opportunity

Planning a collaborative community health assessment and community health improvement plan for your unique community
The Transformation Center is excited to announce a training opportunity for CCOs to partner with local public health and Tribal health departments, hospitals and local mental health authorities to collaboratively develop community health assessments (CHAs) and community health improvement plans (CHPs). This training is built on best practices from several industries; the Mobilizing for Action through Planning and Partnerships model; and previous state CHA/CHP alignment work.
