DUII Treatment Certificate

What is a DUII Treatment Completion Certificate?

Individuals who are convicted of a DUII will need to obtain DMV Form 735-6821 (DUII Treatment Completion Certificate) in order to reinstate their driving privileges. The DUII Treatment Completion Certificate (DTCC) is a four-part form with a unique identification number and is issued only by Division-approved DUII Services Providers or the Division.

How to Obtain a DUII Treatment Completion Certificate

Residents of Oregon will receive a DTCC upon successful completion of a Division-approved DUII services program as outlined in OAR 309-019-0195.

Please note: OHA cannot issue a DTCC for services completed at an agency that is not a Division-approved DUII Services Provider. Please check the Substance Use Disorders Services Directory to ensure the program you complete is provided by an approved DUII Services Provider.

Individuals who need assistance obtaining a DTCC from a current or former Division-approved DUII services provider may request one from OHA by submitting:

Copy of your ADSS Screening & Referral,
Proof of completion of an Oregon DUII program,

Release of Information to DUII Services Provider (English) (Spanish) (Russian) (Vietnamese) (Simplified Chinese)

OHA will issue a DTCC upon receipt of the following documents:

Proof of out-of-state residency,
Copy of your ADSS Screening & Referral,
Copy of your completion certificate from an out-of-state DUII Services Program or a completed DUII Out-of-State Services Completion Verification form (English) (Spanish) (Russian) (Vietnamese) (Traditional or Simplified Chinese)
Release of Information to DMV (English) (Spanish) (Russian) (Vietnamese) (Simplified Chinese)
