PrimaryHealth of Josephine County, LLC

Primary Health of Josephine County serves Oregon Health Plan members in Josephine County and parts of Douglas and Jackson Counties.​

The Oregon Health Authority’s Metrics and Scoring Committee identified 17 outcome and quality metrics to help monitor coordinated care organization (CCO) performance. Additionally, the state is measuring CCOs on 17 state performance metrics. Measure specifications and methodology are available online on the state's metrics page.

This page shows 2011 baseline data and benchmarks for 27 of the 34 metrics. There are two baselines shown here. One shows a statewide average of 2011 data that was taken from the care organizations that existed in Oregon prior to the formation of CCOs; the other is the baseline specific to PrimaryHealth of Josephine County, LLC​, which is what PrimaryHealth of Josephine County, LLC's performance in 2013 will be measured against.
