Healthy Futures: Program Details and How To Participate

Healthy Futures is a program designed to encourage OEBB members to learn their individual health risks and how to take action to reduce or eliminate risks whenever possible.
 
 
Start your journey to a Healthy Future! It’s as easy as …
 
1. Choose to participate
Log into the MyOEBB system anytime between August 15 and March 31 and indicate “YES – I want to participate in Healthy Futures!” If you cover a spouse or domestic partner on your medical plan, both you and your spouse/partner need to participate in order to meet the program requirements.
 
2. Learn your risks
Log into your medical carrier’s website and complete a 100% confidential online health assessment anytime between now and May 31.  If you cover a spouse or domestic partner on your medical plan, both you and your spouse/partner need to complete this step in order to meet the program requirements.  
 
·         Kaiser members can go to http://kp.org/tha and click “Start a Total Health Assessment now.”  Log in to your account, or click “Register now” to create a new account, if needed. To create an account you will need your Member ID that is listed on your Kaiser Permanente medical card. Once you’re logged in, click on “Total Health Assessment” (look for the yellow sunflower at the top of  the page). For assistance, call Kaiser Permanente at 800-813-2000.
·         Moda Health members can go to www.modahealth.com/oebb and log in to your myModa account, or create a new account, if needed. To create an account you will need your Subscriber ID that is listed on your Moda Health medical card. Once you’re logged in, click on the "myHealth" tab, then click the "Momentum, powered by Moda Health" link near the middle of the page, then click the "Visit Momentum now" button at the bottom of the screen. If you cover a spouse/partner on your medical plan, they need to complete their own health assessment as well. Their Subscriber ID will be the same as yours, but they will need to create a unique user ID and  password. For assistance, call Moda Health at 866-923-0409.
 
3. Take two actions
Complete two health-supporting actions by August 15. If you cover a spouse or domestic partner on your medical plan, both you and your spouse/partner need to complete this step in order to meet the program requirements.
 
4. Report your Activities!
OEBB will receive a report from Moda Health and Kaiser Permanente identifying members and spouses/domestic partners who completed Step 2 (indicating completion only, no personal results). During the open enrollment period, log in to the MyOEBB system and record your two healthy actions from Step 3. If you cover a spouse or domestic partner on your medical plan, both you and your spouse/partner need to complete this step in order to meet the program requirements.
 
If you (and your spouse/partner, if you cover one on your medical plan) both complete these four steps, everyone on your OEBB medical plan will receive $100 per person lower deductible*, effective October 1 of the next plan year! That could be a value of up to $300 depending your plan selection and the number of dependents covered.
*Qualifying participants who enroll in Kaiser Medical Plan 1 (the only OEBB medical plan without a deductible) will receive a financially equivalent reduction in copays.
