Oregon Coordinated Care Organizations Transformation Plans

Oregon CCO Transformation Plans

CCO transformation plans establish the foundation for OHA’s partnership with CCOs to achieve Oregon’s health system goals. Plans also encourage continuous quality improvement, recognizing that transformation is an continuous process and that a CCO’s transformation plan will and should evolve over time. As part of the contract process, each CCO was required to develop a transformation plan geared specifically to the needs of the community it serves. Plans demonstrate how the organization will work to improve health outcomes, increase member satisfaction and reduce overall costs.
