The process for recognizing primary care homes is based on the work of the PCPCH Standards Advisory Committee.
The Oregon Health Authority's Office for Oregon Health Policy and Research has adopted permanent administrative rules for the Patient-Centered Primary Care Home Program. The Northwest Health Foundation, in partnership with the Oregon Health Authority, established the Patient-Centered Primary Care Home Implementation Task Force that provided recommendations for broad implementation of primary care homes in Oregon. This is their final report: PCPCH Implementation Task Force Report
The top priority identified by the task force was to build on the expertise and resources in Oregon by creating a Patient-Centered Primary Care Institute to convene and broker resources, create a centralized learning system, and provide technical assistance to clinics working to become recognized.
The Oregon Health Care Quality Corporation was awarded $1.3M to establish the Patient-Centered Primary Care Institute to help clinics improve care and become recognized Patient-Centered Primary Care Homes.
