The Northwest Health Foundation, in partnership with the Oregon Health Authority, established the Patient-Centered Primary Care Home Implementation Task Force that provided recommendations for broad implementation of primary care homes in Oregon. This is their final report: PCPCH Implementation Task Force Report
The top priority identified by the task force was to build on the expertise and resources in Oregon by creating a Patient-Centered Primary Care Institute to convene and broker resources, create a centralized learning system, and provide technical assistance to clinics working to become recognized.
The Oregon Health Care Quality Corporation was awarded $1.3M to establish the Patient-Centered Primary Care Institute to help clinics improve care and become recognized Patient-Centered Primary Care Homes.
