Choosing A FHIAP-Approved Health Plan
Plan Information
An insurance agent can help you choose a health plan from this list. Call FHIAP 888-564-9669 (Toll-free in Oregon) for the name of a producer (agent). This service is free.
 
If you select a plan on your own, call the insurance company for an application. When you complete the insurance company’s application, sign it and attach the Certificate of Eligibility to the application, and send it to the insurance company for approval. The insurance company will notify FHIAP if you are approved.
 
The Certificate of Eligibility tells the health insurance company to bill FHIAP (not you) for your insurance. FHIAP will send you a bill for your portion of the premium.
